摘  要:目的:探讨磁共振成像(MRI)在心肌梗塞支架植入后心功能恢复评估中的价值。方法:回顾性分析2019年10月~2023年5月在本院接受心脏MRI检查的心肌梗塞支架植入术后患者共47例,利用心脏磁共振成像技术获取左右心室射血分数(LVEF和RVEF)、左右心室舒张末期容积指数(LVEDVI和RVEDVI)及收缩末期容积指数(LVESVI和RVESVI)等心功能参数。同时,结合心肌延迟增强(LGE)扫描结果,评估心肌瘢痕负荷及分布情况。结果:支架植入后,患者LVEF值平均提升至(45±10)%,较植入前(28±9)%显著提高(P<0.05);RVEF值亦有所改善,但改善程度较LVEF小;LVEDVI和LVESVI值较植入前显著降低,分别平均下降了30mL/m^(2)和25mL/m^(2),差异有统计学意义(P<0.05),提示左心室容量负荷减轻;RVEDVI和RVESVI值亦有所下降,但变化不如左心室显著。心肌延迟增强扫描显示,心肌瘢痕负荷较植入前减少,且瘢痕分布更趋于局限。进一步分析发现,心功能恢复程度与心肌瘢痕负荷减少量呈正相关(r=0.45,P<0.05)。结论:磁共振成像能够精准评估心肌梗塞支架植入后患者的心功能恢复情况,通过定量分析心脏结构和功能参数,为临床提供客观、全面的评估依据。Objective:To investigate the value of magnetic resonance quantitative imaging(MRI)in the assessment of cardiac function recovery after myocardial infarction(MI)stent implantation.Methods:A retrospective analysis was conducted on 47 patients who underwent MRI examination after MI stent implantation in our hospital from October 2019 to May 2023.Cardiac magnetic resonance imaging(CMR)technology was utilized to obtain cardiac function parameters such as bilateral ventricular ejection fractions(LVEF and RVEF),bilateral ventricular end-diastolic volume indexes(LVEDVI and RVEDVI),and end-systolic volume indexes(LVESVI and RVESVI).Additionally,myocardial scar burden and distribution were evaluated based on myocardial late gadolinium enhancement(LGE)scan results.Results:After stent implantation,patients'LVEF values significantly increased to an average of(45±10)%,compared to(28±9)%before implantation(P<0.05).RVEF values also improved,albeit to a lesser extent compared to LVEF.LVEDVI and LVESVI values significantly decreased by an average of 30mL/m²and 25mL/m²,respectively(P<0.05),indicating a reduction in left ventricular volume load.RVEDVI and RVESVI values also decreased,although the changes were less pronounced than those in the left ventricle.LGE scans revealed a reduction in myocardial scar burden compared to preimplantation,with scars becoming more localized.Further analysis showed a positive correlation between the degree of cardiac function recovery and the reduction in myocardial scar burden(r=0.45,P<0.05).Conclusion:Magnetic resonance quantitative imaging can accurately assess cardiac function recovery in patients after MI stent implantation.By quantitatively analyzing cardiac structure and function parameters,it provides an objective and comprehensive basis for clinical assessment.
