经阴道彩色多普勒超声诊断异位妊娠的临床价值

摘　　要：目的:通过回顾性分析异位妊娠(EP)患者的临床资料,探讨经阴道彩色多普勒超声(TVS)在EP诊断中的应用价值。方法:选取2021年1月~2023年7月在本院产检的130例EP患者作为研究对象,患者年龄21~49岁,初产妇15例,经产妇115例。所有患者均进行TVS检查,分析TVS检查结果与术后病理结果的符合率以及EP患者的超声声像图表现。结果:比较分析TVS检查与术后病理结果的符合情况,结果发现,经TVS对异位妊娠类型检查结果与术后病理检查的符合率较高,两者无显著差异(P>0.05)。基于超声检查声像图,发现经阴道彩色多普勒超声可以清晰地观察到异位孕囊阴影、有无混合肿块,以及胎儿心脏和胚胎、附件区和卵黄囊的存在。结论:经阴道彩色多普勒超声在异位妊娠诊断方面作用显著,且图像质量较好。但临床上应避免对合并阴道及宫内炎症的患者实施该检查,以避免此类患者感染加重。因此,在临床实践中,EP的临床诊断需要根据患者的实际情况选择最适合的检测方法。Objective:To evaluate the value of transvaginal color Doppler ultrasonography(TVS)in the diagnosis of ectopic pregnancy(EP)by retrospective analysis of the clinical data of EP patients.Methods:A total of 130 EP patients,aged 21-49 years old,15 primipara and 115 menpara,were selected from our hospital from January 2021 to July 2023.TVS examination was performed in all patients,and the coincidence rate of TVS examination results and postoperative pathological results as well as the ultrasonographic manifestations of EP patients were analyzed.Results:The conformity of TVS examination and postoperative pathological examination was compared and analyzed.The results showed that the conformity rate of TVS examination for ectopic pregnancy type was higher than that of postoperative pathological examination,and there was no significant difference between the two(P>0.05).Based on ultrasonography,it was found that transvaginal color Doppler ultrasound could clearly observe the ectopic pregnancy sac shadow,the presence of mixed mass,the presence of fetal heart and embryo,the accessory area and the yolk sac.Conclusion:Transvaginal color Doppler ultrasound plays a significant role in the diagnosis of ectopic pregnancy,and the image quality is good.However,this examination should be avoided clinically in patients with vaginal and intrauterine inflammation to avoid further aggravation of infection in such patients.Therefore,in clinical practice,the clinical diagnosis of EP needs to choose the most suitable detection method according to the actual situation of patients.
