可视喉镜在院前急救气管插管中的应用及临床价值

摘　　要：目的:探析可视喉镜在院前急救气管插管中的应用效果及临床价值。方法:选择2023年1月~2024年6月本院急诊收治的100例院前急救气管插管患者,按使用喉镜不同分为两组,各50例,对照组采用传统光学喉镜,观察组采用可视喉镜,比较两组插管情况、血流动力学指标、血清指标、并发症发生率。结果:观察组一次插管成功率更高,插管时间以及声门暴露时间均更短(P<0.05)。与对照组比较,插管时、插管后,观察组HR、MAP、RPP波动幅度更小(P<0.05)。插管后,观察组Cor、IL-8均更低(P<0.05)。观察组呛咳、咽喉部渗血、喉部痉挛、口腔创伤等并发症总发生率6.00%更低(P<0.05)。结论:在院前急救气管插管中应用可视喉镜,可进一步提升一次插管成功率,缩短插管时间以及声门暴露时间,同时还能更好维持患者生命体征稳定,减少并发症发生。Objective:To explore the application effect and clinical value of visual laryngoscope in pre hospital emergency tracheal intubation.Methods:100 pre hospital emergency endotracheal intubation patients admitted to our hospital from January 2023 to June 2024 were selected and divided into two groups,with 50 patients in each group,based on the use of laryngoscopes.The control group used traditional optical laryngoscope,while the observation group used visual laryngoscope.The intubation situation,hemodynamic indicators,serum indicators,and incidence of complications were compared between the two groups.Results:The observation group had a higher success rate of intubation,shorter intubation time,and shorter glottal exposure time(P<0.05).Compared with the control group,the observation group had smaller fluctuations in HR,MAP,and RPP during and after intubation(P<0.05).After intubation,the observation group had lower levels of Cor and IL-8(P<0.05).The total incidence of complications such as coughing,throat bleeding,throat spasms,and oral trauma in the observation group was lower at 6.00%(P<0.05).Conclusion:The application of visual laryngoscope in pre hospital emergency tracheal intubation can further improve the success rate of intubation,shorten the intubation time and glottal exposure time,and better maintain the stability of patients’vital signs,reducing the occurrence of complications.
