孤独症谱系障碍患儿照顾者内化污名的影响因素分析

摘　　要：目的调查分析孤独症谱系障碍(ASD)患儿家庭照顾者内化污名相关因素。方法选取2023年1—10月于医院就诊的200例孤独症谱系障碍患儿家属作为调查对象。调查患儿及家属一般资料、家庭复原力、特殊家庭赋能和家庭照顾者内化污名情况;Pearson相关性分析ASD患儿家庭照顾者内化污名与家庭复原力和特殊家庭赋能的关系;多元逐步线性回归分析ASD患儿照顾者内化污名的影响因素。结果ASD患儿家庭复原力平均得分为8.29±17.32分,特殊家庭赋能平均得分为75.94±13.97分,连带病耻感量表(ASS)平均得分为44.35±10.84分。Pearson相关性分析结果显示,ASD患儿家庭照顾者内化污名与家庭复原力量表得分呈负相关,与特殊家庭赋能量表得分呈正相关(r=-0.668,P<0.05;r=0.722,P<0.05)。多元线性逐步回归结果显示,患儿的年龄、照顾者的文化程度、父母婚姻、家庭月收入、对患儿孤独症的认知程度是患儿照顾者内化污名发生的影响因素(P<0.05)。结论ASD患儿家庭照顾者内化污名与家庭复原力量表得分呈负相关,与特殊家庭赋能量表得分呈正相关。患儿年龄、照顾者文化程度、照顾者婚姻状况、家庭月收入和对患儿孤独症的认知程度均会对内化污名情况造成影响。Objective To investigate and analyse the factors related to internalised stigma among family caregivers of children with autism spectrum disorder(ASD).Methods The families of 200 children with autism spectrum disorder(ASD)admitted to the hospital from January to October 2023 were selected as survey subjects.General information,family resilience,special family empowerment,and internalised stigma of family caregivers of children with ASD were investigated.Pearson correlation analysis was conducted to analyse the relationship between internalised stigma of family caregivers of children with ASD,family resilience,and special family empowerment,multivariate stepwise linear regression analysis was conducted to analyse the influencing factors of internalised stigma of caregivers of children with ASD.Results The mean family resilience score for children with ASD was 8.29±17.32 points,the average special family empowerment score was 75.94±13.97 points,and the mean Associated Stigma Scale(ASS)score was 44.35±10.84 points.Pearson correlation analyses showed a negative correlation between internalised stigma and family resilience scores for caregivers of families of children with ASD,and a positive correlation with special family empowerment scores for children with ASD.Special Family Empowerment Scale scores were positively correlated(r=-0.668,P<0.05;r=0.722,P<0.05).The results of multiple linear stepwise regression showed that the age of the child,the literacy level of the caregiver,parental marriage,monthly family income,and the level of awareness of the child's autism were the influencing factors in the occurrence of internalised stigma among the child's caregivers(P<0.05).Conclusion Internalised stigma among family caregivers of children with ASD was negatively correlated with scores on the Family Resilience Scale and positively correlated with scores on the Special Family Empowerment Scale.Child's age,caregiver's edncation level,caregiver's marital status,monthly family income,and knowledge of the child's autism aff
