“3S2E”护理管理模式联合下颌收拢抗阻力运动在老年缺血性脑梗死吞咽障碍患者中的应用

摘　　要：目的探讨“3S2E”护理管理模式联合下颌收拢抗阻力运动在老年缺血性脑梗死吞咽障碍患者中的应用效果。方法选取2022年12月—2023年6月宁夏医科大学总医院神经内科住院治疗且符合纳入条件的96例老年缺血性脑梗死吞咽障碍患者为研究对象,按照组间基本资料具有可比性的原则分为对照组和观察组,每组48例,本研究实施过程中,对照组1例患者出院后死亡退出研究,最终观察组48例和对照组47例患者完成研究。对照组接受药物治疗及常规护理,观察组在对照组基础上实施“3S2E”护理管理模式联合下颌收拢抗阻力运动,主要包括提高护理技能(护士业务技能、患者及家属护理技能)、强化护士服务意识、分层分级管理保障护理安全、对患者进行吞咽功能评估及落实健康教育。比较两组患者干预前后吞咽功能、营养状态、误吸情况,记录两组患者护理满意程度。结果干预4周后,观察组患者的吞咽功能评分低于对照组,差异有统计学意义(P<0.05);干预4周后,观察组患者的血红蛋白、血清白蛋白均高于对照组,差异有统计学意义(P<0.05);干预4周后,观察组患者误吸发生率低于对照组,差异有统计学意义(P<0.05);观察组患者对临床护理满意程度高于对照组,差异有统计学意义(P<0.05)。结论“3S2E”护理管理模式联合下颌收拢抗阻力运动是一种有效的护理方法,它结合了运动训练和护理管理两个方面的优势,可以改善患者吞咽功能和营养状况,减少误吸发生率,增强其生活质量,提高患者对临床护理工作的满意程度,提升患者就医体验。Objective To investigate the application effect of"3S2E"nursing management model combined with chin tuck against resistance exercise in elderly patients with ischemic cerebral infarction and dysphagia.Methods A total of 96 elderly patients with ischemic cerebral infarction and dysphagia,who were hospitalized in the Department of Neurology at General Hospital of Ningxia Medical University from December 2022 to June 2023 and met the inclusion and exclusion criteria,were selected as the research subjects.According to the principle of comparability of basic data between the groups,they were divided into a control group and an observation group,with 48 cases in each group.During the implementation of this study,one patient in the control group died after discharge and was withdrawn from the study.Finally,48 patients in the observation group and 47 patients in the control group completed the study.The control group received drug treatment and routine nursing care.The observation group,on the basis of the control group,received the"3S2E"nursing management model combined with chin tuck against resistance exercise,which primarily included improving nursing skills(nurses'professional skills,as well as the nursing skills of patients and their families),enhancing nurses'service awareness,implementing stratified and hierarchical management to ensure nursing safety,conducting swallowing function assessments for patients,and implementing health education.The swallowing function,nutritional status and aspiration of both groups of patients were compared before and after the intervention,and the nursing satisfaction levels of both groups were recorded.Results After 4 weeks of intervention,the swallowing function score in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).After 4 weeks of intervention,the hemoglobin and serum albumin in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).After 4 week
