卡瑞利珠单抗致低血压休克一例

摘　　要：本文报道1例肝癌患者接受常规注射剂量的卡瑞利珠单抗后,出现低血压休克的严重不良反应,给予紧急的个体化救治,患者抢救成功。经诺氏评估量表评价,该患者的低血压休克很可能由卡瑞利珠单抗导致,分析可能的机制是由肌酸激酶同工酶异常升高介导的心肌损伤所致。提示临床实践使用卡瑞利珠单抗,需要密切监护患者血压、心率、心电图及肌酸激酶同工酶等心脏功能指标,注意个体化情况,警惕出现低血压休克,确保患者用药安全。This paper reports a case of severe adverse reaction of hypotensive shock in one liver cancer patient after receiving a conventional injection dose of camrelizumab.Emergency individualized treatment was given and the rescue was successful.The evaluation result using Naranjo's ADR evaluation scale suggests that the hypotensive shock suffered by the patient was probably caused by camrelizumab,and there was evidence to support the correlation between the ADR and the drug.An analysis of the mechanisms indicates that camrelizumab-induced hypotensive shock is probably caused by myocardial injury mediated by abnormal increase of creatine kinase-myocardial band isoenzyme(CK-MB).In clinical practice,the use of camrelizumab requires close monitoring of cardiac function indicators of patients,such as blood pressure,heart rate,electrocardiogram and CK-MB.It is necessary to pay attention to the individual situation and be alert to the occurrence of hypotensive shock,thus to ensure the medication safety of patients.
