VVST-CV联合面部吞咽训练在脑卒中吞咽障碍患者中的应用效果

摘　　要：目的探讨改良容积黏度测试(volume viscosity swallowing test-Chinese version,VVST-CV)联合面部吞咽训练在脑卒中吞咽障碍患者中的应用效果。方法选取2022年3月—2023年12月于本院接受治疗的126例脑卒中吞咽障碍患者为研究对象,采用随机数字表法将患者分为对照组(63例)和观察组(63例)。对照组实施常规护理,观察组实施VVST-CV联合面部吞咽训练。比较两组患者的吞咽功能、吞咽障碍情况、营养状况、并发症发生率及生活质量。结果护理后,观察组患者的标准吞咽功能评估量表(Standardized Swallowing Assessment,SSA)评分低于对照组(P<0.05),藤岛一郎吞咽评分高于对照组,差异有统计学意义(P<0.05);观察组患者的白蛋白、血红蛋白以及总蛋白水平均高于对照组,差异有统计学意义(P<0.05);观察组患者的并发症总发生率低于对照组,差异有统计学意义(P<0.05);观察组患者的生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论对脑卒中吞咽障碍患者实施VVST-CV联合面部吞咽训练,可有效改善患者的吞咽障碍症状,加快患者的吞咽功能恢复速度,改善患者康复过程中的营养状况,降低并发症的发生风险,提高患者的生活质量。Objective To explore the application effect of modified Volume Viscosity Swallowing Test-Chinese Version(VVST-CV)combined with facial swallowing training in stroke patients with dysphagia.Methods A total of 126 stroke patients with dysphagia treated in our hospital from March 2022 to December 2023 were selected and divided into a control group(63 cases)and an observation group(63 cases)using a random number table.The control group received routine nursing care,while the observation group received VVST-CV combined with facial swallowing training.The swallowing function,degree of dysphagia,nutritional status,incidence of complications,and quality of life were compared between the two groups.Results After nursing,the Standardized Swallowing Assessment(SSA)score of the observation group was lower than that of the control group(P<0.05).The Fujishima Swallowing Score of the observation group was higher than that of the control group,with a statistically significant difference(P<0.05).The levels of albumin,hemoglobin,and total protein in the observation group were significantly higher than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,with a statistically significant difference(P<0.05).The quality of life score of the observation group was higher than that of the control group,with a statistically significant difference(P<0.05).Conclusion The implementation of VVST-CV combined with facial swallowing training in stroke patients with dysphagia can effectively improve their dysphagia symptoms,accelerate the recovery of swallowing function,enhance nutritional status during recovery,reduce the risk of complications,and improve their quality of life.
