从左归丸、右归丸浅窥张介宾真阴论

摘　　要：真阴理论是张介宾(号景岳)所提出的用于揭示人体以真阴为生命活动物质基础的本质,并应用于临床阐释病机、诊疗疾病和组方用药的重要学说。景岳所论之真阴能化生为元阴、元阳,元阴化为五脏六腑之精气以滋养人体,元阳通过推动全身气机运动、不断温煦脏腑而使机体能够发挥正常的生命活动,故真阴亏损,则五脏六腑失于温煦、濡养,从而发生一系列的病理变化。该理论至今仍是临床医家学习探究的重点,但其机制复杂,内容广泛,临床价值尚未完全体现。左归丸、右归丸是张介宾真阴理论所得之核心方,也是后世理解其真阴学说的关键,前者为纯甘壮水之剂,后者为益火填精之剂,虽在治法上相反,但在用药上均重用填补真阴之熟地黄,在组方上用甘温益火之剂补阳以配阴、纯甘壮水之剂补阴以配阳,通过运用阴阳互求的处方思想,揭示了阴虚、阳虚均为真阴亏损的本质,由此体现阴阳一体,本为真阴的思想。研究通过探讨张介宾之左归丸、右归丸的药物配伍特点,以阐释景岳真阴理论的学术思想,以及“填补真阴、阴阳互求”的立方基础,以期为日后临床工作者应用“真阴”理论在临床诊疗过程中辨证论治和组方加减提供理论依据。True-yin theory is an important theory put forward by ZHANG Jiebin,which is used to reveal the essence of human body based on true-yin as the material basis of life activities,and is applied to clinical explanation of pathogenesis,diagnosis and treatment of diseases,and formulation of medicine.According to ZHANG Jiebin,true-yin is transformed into original Yin(阴)and original Yang(阳).Original Yin can be transformed into the vital Qi(气)of the five viscera and six fu-organs to nourish the human body.Original Yang can constantly warm the viscera and make the body play normal life activities by promoting the Qi movement of the whole body,so the deficiency of true-yin will cause the five viscera and six fu-organs to lose warmth and nourishmet,resulting in a series of pathological changes.This theory is still the focus of the study and exploration of clinicians,but its mechanism is complex,the content is extensive,and the clinical value has not been fully reflected.Zuogui pills(左归丸)and Yougui pills(右归丸)are the core formula originated from ZHANG Jiebin’s true-yin theory,and also the key to later generations’understanding of his true-yin theory.The former is the agent of reinforcing water,while the latter is the agent of supplementing fire and filling essence.Although they are opposite in treatment methods,they both make full use of Radix Rehmanniae Preparata to fill true-yin.In the composition of the prescription,the agent of warming Yang and fire supplementing is applied to match Yin,and the agent of reinforcing water and supplementing Yin is applied to match Yang,through the prescription of Yin and Yang in seeking each other,Yin deficiency and Yang deficiency are revealed as the essence of true-yin deficiency,which reflects the unity of Yin and Yang and the thought of true-yin.By discussing the drug compatibility characteristics of Zuogui pills()and Yougui pills of ZHANG Jiebin’s famous prescription,this paper will explain the academic thought of ZHANG Jiebin’s true-yin theory and the prescrip
