摘  要:目的:探讨推拿对神经病理性疼痛大鼠脊髓背角突触相关蛋白Wnt和β-catenin表达的影响。方法:24只SD大鼠随机分为三个组别:空白组、模型组和推拿组,每组8只。运用坐骨神经慢性压迫损伤(CCI)大鼠模型,在造模后第4天开始对推拿组大鼠进行按揉法干预,持续14天。空白组和模型组不进行推拿干预。通过测量造模前及造模后第4、10、14、17天大鼠机械缩足阈值(PWT)、热痛阈值(PWL)来评估大鼠的疼痛行为变化。HE染色观察右侧脊髓背角神经元形态变化,利用免疫荧光染色法检测β-catenin蛋白的表达,使用Western blot方法检测Wnt和β-catenin蛋白的表达水平。结果:行为学结果显示,模型组大鼠的PWL和PWT显著降低,提示疼痛敏感性增加。经过推拿干预后,推拿组大鼠的PWL和PWT显著提高,表明推拿能够有效缓解神经病理性疼痛(P<0.001);免疫荧光染色结果显示,与模型组相比,推拿组大鼠右侧脊髓背角β-catenin阳性表达减少(P<0.001);HE染色显示,模型组的右侧脊髓背角神经元损伤严重部分神经元胞体出现萎缩,核的形态略显不规则,组织结构松散,可见间隙增多;推拿组的神经元损伤有所减轻,部分神经元大小趋于恢复,胞体形态较为饱满,背景组织结构较为清晰,细胞排列较为整齐,间隙有所减少。Western Blot结果显示,CCI大鼠右侧脊髓背角Wnt、β-catenin蛋白表达增加,推拿组Wnt、β-catenin蛋白表达减少(P<0.01、P<0.001)。结论:推拿能够减轻神经病理性疼痛模型大鼠脊髓背角的神经元损伤,其缓解疼痛的作用机制可能涉及下调Wnt及β-catenin的表达。Objective:To investigate the effect of Tuina therapy on the expression of synaptic proteins Wnt andβ-catenin in the spinal dorsal horn of rats with neuropathic pain.Methods:Twenty-four SD rats were randomly divided into three groups:blank group,model group,and Tuina group,with eight rats in each group.The chronic constrictive injury(CCI)model was used to induce neuropathic pain in rats.Starting on the 4th day after modeling,rats in the Tuina group received kneading and rubbing interventions for 14 consecutive days.No intervention was applied to the blank and model groups.Pain behavioral changes were assessed by measuring paw withdrawal threshold(PWT)and paw withdrawal latency(PWL)on the 4th,10th,14th,and 17th days post-modeling.Hematoxylin-eosin(HE)staining was used to observe morphological changes in neurons in the right spinal dorsal horn.β-catenin protein expression was detected via immunofluorescence staining,and Western blot was used to analyze the expression levels of Wnt andβ-catenin proteins.Results:Behavioral results showed that the PWL and PWT of rats in the model group were significantly reduced,indicating increased pain sensitivity.After Tuina intervention,the PWL and PWT of rats in the Tuina group were significantly improved,demonstrating that Tuina effectively alleviates neuropathic pain(P<0.001).Immunofluorescence staining showed thatβ-catenin positive expression in the right dorsal horn of the spinal cord was reduced in the Tuina group compared to the model group(P<0.001).HE staining revealed severe neuronal damage in the right dorsal horn of the spinal cord in the model group,with some neuronal cell bodies showing shrinkage,slightly irregular nuclear morphology,and loose tissue structure with increased intercellular spaces.In the Tuina group,neuronal damage was alleviated,with some neurons showing partial recovery in size,more intact and plump cell bodies,clearer background tissue structure,more organized cell arrangement,and reduced intercellular spaces.Western blot results indicated increas
