复方血栓通胶囊联合地尔硫?治疗气阴两虚型微血管性心绞痛的临床研究

摘　　要：目的探讨复方血栓通胶囊联合地尔硫?片治疗气阴两虚型微血管性心绞痛的临床疗效。方法选取2022年6月—2023年12月在河南中医药大学第三附属医院治疗的气阴两虚型微血管性心绞痛患者81例,根据不同治疗方案分成对照组(40例)和治疗组(41例)。对照组患者口服盐酸地尔硫?片,30 mg/次,3次/d。治疗组在对照组基础上口服复方血栓通胶囊,3粒/次,3次/d。两组患者均进行6周的治疗。观察两组患者临床疗效,比较治疗前后两组患者中医证候评分和Duke评分,全血高切黏度、全血低切黏度和血浆黏度,左心室射血分数(LVEF)、6 min步行距离(6MWT)、内皮素-1(ET-1)、超敏C反应蛋白(hs-CRP)和低密度胆固醇(LDL-C)水平。结果治疗后,对照组和治疗组总有效率分别为75.00%和95.12%,两组比较差异具有统计学意义(P<0.05)。经治疗,两组中医证候评分降低,而Duke评分升高(P<0.05),且治疗后治疗组中医证候评分低于对照组,Duke评分高于对照组(P<0.05)。经治疗,两组患者LVEF和6MWT水平比治疗前明显升高(P<0.05),且治疗组较对照组升高更明显(P<0.05)。经治疗,两组患者全血高切黏度、全血低切黏度和血浆黏度均较治疗前明显降低(P<0.05),且治疗组在各指标较对照组降低更为显著(P<0.05)。经治疗,两组患者血清ET-1、hs-CRP和LDLC水平均较治疗前明显降低(P<0.05),且治疗组各指标较对照组下降更显著(P<0.05)。结论复方血栓通胶囊联合地尔硫?片治疗气阴两虚型微血管性心绞痛疗效显著,不仅能够显著改善患者心绞痛症状、疾病严重程度和血液流变学指标,同时还能够提升患者心功能状态。Objective Exploring the efficacy of Compound Xueshuantong Capsules combined with diltiazem in treatment of microvascular angina pectoris with Qiyin deficiency type.Methods Patients(81 cases)with microvascular angina pectoris with Qiyin deficiency type in the Third Affiliated Hospital of Henan University of Traditional Chinese Medicine from June 2022 to December 2023 were divided into control(40 cases)and treatment(41 cases)group based on different treatments.Patients in the control group were po administered with Diltiazem Hydrochloride Tablets,30 mg/time,three times daily.Patients in the treatment group were po administered with Compound Xueshuantong Capsules,3 grains/time,three times daily.Patients in two groups were treated for 6 weeks.After treatment,the clinical evaluations were evaluated,the traditional Chinese medicine syndrome score and Duke score,whole blood high shear viscosity,whole blood low shear viscosity and plasma viscosity,the levels of LVEF,6MWT,ET-1,hs-CRP and LDL-C in two groups before and after treatment were compared.Results After treatment,the total clinical effective rates in the control group and the treatment group were 75.00%and 95.12%,respectively,and the difference between the two groups was statistically significant(P<0.05).After treatment,TCM syndrome score decreased in both groups,but Duke score increased(P<0.05).After treatment,TCM syndrome score in the treatment group was lower than that in control group,Duke score was higher than that in control group(P<0.05).After treatment,LVEF and 6MWT levels in two groups were significantly higher than before treatment(P<0.05),and the increase in the treatment group was more significant than that in the control group(P<0.05).After treatment,the whole blood highshear viscosity,whole blood low-shear viscosity and plasma viscosity were significantly lower than before treatment(P<0.05),and the reduction in each index in the treatment group was more significant than that in the control group(P<0.05).After treatment,the serum ET-1,hs-CRP and LDL-C
