达格列净联合氯沙坦钾与沙库巴曲缬沙坦钠治疗心力衰竭合并高血压患者的效果

摘　　要：目的:观察达格列净联合氯沙坦钾与沙库巴曲缬沙坦钠治疗心力衰竭合并高血压患者的效果。方法:选取2022年5月至2023年10月该院收治的84例心力衰竭合并高血压患者进行前瞻性研究,按照随机数字表法将其分为对照组与观察组各42例。对照组采用氯沙坦钾与沙库巴曲缬沙坦钠治疗,观察组在对照组基础上联合达格列净治疗。比较两组治疗前后心功能指标[左心室射血分数(LVEF)、心输出量(CO)、每搏输出量(SV)]、血压[舒张压(DBP)、收缩压(SBP)]、心率、血清标志物[脂蛋白相关磷脂酶A2(Lp-PLA2)、血管紧张素Ⅱ(AngⅡ)]水平,以及治疗期间不良反应发生率。结果:治疗后,两组LVEF、CO、SV水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);治疗后,两组SBP、DBP、心率水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组Lp-PLA2、AngⅡ水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:达格列净联合氯沙坦钾与沙库巴曲缬沙坦钠治疗心力衰竭合并高血压患者可提高心功能指标水平,降低血清标志物、血压和心率水平,效果优于氯沙坦钾与沙库巴曲缬沙坦钠治疗。Objective:To observe effects of Dapagliflozin combined with Losartan potassium and Sacubitril Valsartan sodium in treatment of patients with heart failure and hypertension.Methods:A prospective study was conducted on 84 patients with heart failure and hypertension admitted to this hospital from May 2022 to October 2023.According to the random number table method,they were divided into control group and observation group,42 cases in each group.The control group was treated with Losartan potassium and Sacubitril Valsartan sodium,while the observation group was treated with Dapagliflozin on the basis of that of the control group.The levels of cardiac function indexes [left ventricular ejection fraction(LVEF),cardiac output(CO),stroke volume(SV)],blood pressure [diastolic blood pressure(DBP),systolic blood pressure(SBP)],heart rate and serum marker indexes [lipoprotein-associated phospholipase A2(Lp-PLA2),angiotensin II(Ang II)] before and after the treatment,and the incidence of adverse reactions during treatment were compared between the two groups.Results:After the treatment,the levels of LVEF,CO and SV in the two groups were higher than those before the treatment,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of SBP,DBP and heart rate in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of Lp-PLA2 and Ang II in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Dapagliflozin combined with Losartan potassium tablets and Sacubitril Valsartan sodium in the treatment of th
