左炔诺孕酮宫内释放系统联合戈舍瑞林治疗子宫内膜异位症术后患者的效果

摘　　要：目的:观察左炔诺孕酮宫内释放系统联合戈舍瑞林治疗子宫内膜异位症术后患者的效果。方法:回顾性分析2020年9月至2022年9月该院收治的95例子宫内膜异位症患者的临床资料,按照治疗方法不同将其分为对照组(n=47)和观察组(n=48)。对照组采用戈舍瑞林治疗,观察组在对照组基础上联合左炔诺孕酮宫内释放系统治疗。比较两组治疗前后卵巢功能指标[抗米勒管激素(AMH)、雌二醇(E_(2))、促黄体生成素(LH)、促卵泡激素(FSH)]水平、痛经持续时间、疼痛程度[视觉模拟评分法(VAS)],以及不良反应发生率、复发率。结果:治疗3个月,两组AMH、E_(2)、LH、FSH水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组痛经持续时间均短于治疗前,且观察组短于对照组,两组VAS评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);随访1年,观察组复发率低于对照组,差异有统计学意义(P<0.05)。结论:左炔诺孕酮宫内释放系统联合戈舍瑞林治疗子宫内膜异位症术后患者可降低卵巢功能指标水平、VAS评分和复发率,缩短痛经持续时间,效果优于单纯戈舍瑞林治疗。Objective:To observe effects of Levonorgestrel-releasing intrauterine system combined with Goserelin in treatment of postoperative patients with endometriosis.Methods:The clinical data of 95 patients with endometriosis admitted to this hospital from September 2020 to September 2022 were retrospectively analyzed.According to the different treatment methods,they were divided into control group(n=47)and observation group(n=48).The control group was treated with Goserelin,while the observation group was treated with Levonorgestrel-releasing intrauterine system on the basis of that of the control group.The levels of ovarian function indexes [anti-millerian hormone(AMH),estradiol(E_(2)),luteinizing hormone(LH),follicle stimulating hormone(FSH)] before and after treatment,the duration of dysmenorrhea,the pain degree [visual analogue scale(VAS)],the incidence of adverse reactions,and the recurrence rate were compared between the two groups.Results:After 3 months of treatment,the levels of AMH,E_(2),LH and FSH in the two groups were lower than those before treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the duration of dysmenorrhea in the two groups was shorter than that before treatment,and that in the observation group was shorter than that in the control group;the VAS scores of the two groups were lower than those before treatment,and that in the observation group was lower than that in the control group;and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).After 1year of follow-up,the recurrence rate of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusions:Levonorgestrel-releasing intrauterine system combined with Goserelin in the treatment of the postoperative patients with endometriosis can reduce the levels of ovarian
