腹腔镜下输卵管抽芯切除术与输卵管切除术治疗输卵管妊娠患者的效果比较

摘　　要：目的:比较腹腔镜下输卵管抽芯切除术与输卵管切除术治疗输卵管妊娠患者的效果。方法:回顾性分析2021年8月至2023年5月该院收治的97例输卵管妊娠患者的临床资料,按照手术方法不同将其分为观察组(n=50)与对照组(n=47)。观察组采用腹腔镜下输卵管抽芯切除术治疗,对照组采用腹腔镜下输卵管切除术治疗。比较两组围手术期指标(手术时间、住院时间、出血量、首次下床时间、肠鸣音恢复时间)水平,手术前后卵巢功能指标[黄体生成素(LH)、卵泡刺激素(FSH)、窦卵泡计数(AFC)]、生化指标[抗米勒管激素(AMH)、β-人绒毛膜促性腺激素(β-HCG)、孕酮]水平,以及并发症发生率、术后12个月正常妊娠率。结果:两组手术时间比较,差异无统计学意义(P>0.05);观察组肠鸣音恢复时间、首次下床时间、住院时间均短于对照组,出血量少于对照组,差异有统计学意义(P<0.05);术后3 d,两组LH、FSH水平均低于术前,且观察组低于对照组,两组AFC均多于术前,且观察组多于对照组,差异有统计学意义(P<0.05);术后3 d,两组AMH、β-HCG、孕酮水平均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05);术后12个月,两组并发症发生率和正常妊娠率比较,差异均无统计学意义(P>0.05)。结论:腹腔镜下输卵管抽芯切除术治疗输卵管妊娠患者可缩短住院时间,减少术中出血量,改善卵巢功能指标水平,以及降低生化指标水平,效果优于腹腔镜下输卵管切除术治疗。Objective:To compare effects of laparoscopic core-pulling salpingectomy and salpingectomy in treatment of patients with tubal pregnancy.Methods:The clinical data of 97 patients with tubal pregnancy admitted to this hospital from August 2021 to May 2023 were retrospectively analyzed.According to the different surgical methods,they were divided into observation group(n=50) and control group(n=47).The observation group was treated with laparoscopic core-pulling salpingectomy,while the control group was treated with laparoscopic salpingectomy.The perioperative indexes(operation time,hospitalization time,blood loss,first out of bed time,recovery time of bowel sounds) levels,the ovarian function indexes [luteinizing hormone(LH),follicle stimulating hormone(FSH),antral follicle count(AFC)] levels,the biochemical indexes [anti-Müllerian hormone(AMH),β-human chorionic gonadotropin(β-HCG),progesterone] levels before and after the surgery,the incidence of complications,and the normal pregnancy rate 12 months after the surgery were compared between the two groups.Results:There was no significant difference in the operation time between the two groups(P>0.05).The recovery time of bowel sounds,the first out of bed time and the hospitalization time in the observation group were shorter than those in the control group,the blood loss was less than that in the control group,and the differences were statistically significant(P<0.05).Three days after the surgery,the levels of LH and FSH in the two groups were lower than those before the surgery,and those in the observation group were lower than those in the control group;the AFC levels in the two groups were higher than those before the surgery,and that in the observation group was higher than that in the control group;and the differences were statistically significant(P<0.05).Three days after the surgery,the levels of AMH,β-HCG and progesterone in the two groups were lower than those before the surgery,those in the observation group were lower than those in the control group,an
