钢板螺钉内固定技术治疗四肢长管骨创伤骨折患者的价值研究

摘　　要：目的探讨钢板螺钉内固定技术对四肢长管骨创伤骨折患者的治疗效果。方法60例四肢长管骨创伤骨折患者,随机分为观察组(钢板螺钉内固定技术)和对照组[股骨近端抗旋髓内钉(PFNA)固定],各30例。比较两组临床指标、并发症发生情况、生活质量评分及肢体功能、疼痛评分。结果观察组手术时间、术后早期负重时间、骨折愈合时间、住院时间均短于对照组,出血量少于对照组,组间比较存在统计学意义(P<0.05)。治疗后,观察组肢体功能评分(89.23±3.48)分高于对照组的(73.83±3.10)分,疼痛评分(2.27±0.10)分低于对照组的(4.04±0.20)分,组间比较存在统计学意义(P<0.05)。观察组并发症发生率3.33%(1/30)低于对照组的23.33%(7/30),组间比较存在统计学意义(P<0.05)。治疗后,观察组总体健康、精力、社会功能、心理健康、躯体疼痛、躯体角色、情绪角色、躯体功能评分分别为(63.26±4.49)、(74.26±4.55)、(83.66±3.92)、(81.49±4.26)、(74.49±6.39)、(73.53±4.19)、(84.53±3.68)、(71.38±4.29)分,均高于对照组的(54.55±3.26)、(61.23±4.02)、(76.33±2.76)、(61.25±3.76)、(66.59±4.71)、(61.23±4.43)、(73.06±2.51)、(52.30±2.77)分,组间比较存在统计学意义(P<0.05)。结论钢板螺钉内固定技术对四肢长管骨创伤骨折患者有良好的效果,价值较高。Objective To explore the therapeutic effect of internal fixation with steel plate and screw in the treatment of patients with traumatic fractures of long tubular bones of the extremities.Methods 60 patients with traumatic fractures of long tubular bones of the extremities were randomly divided into an observation group(internal fixation with steel plate and screw)and a control group[proximal femoral nail anti rotation(PFNA)fixation],each with 30 cases.The clinical indicators,complications,quality of life score,extremity function and pain score were compared between the two groups.Results The operation time,early postoperative weight-bearing time,fracture healing time and hospital stay in the observation group were shorter than those in the control group,and the blood loss was less than that in the control group.There was statistical significance between the two groups(P<0.05).After treatment,the observation group had higher extremity function score of(89.23±3.48)points than(73.83±3.10)points in the control group,and lower pain score of(2.27±0.10)points than(4.04±0.20)points in the control group.There was statistical significance between the two groups(P<0.05).The incidence of complications in the observation group was 3.33%(1/30),which was lower than 23.33%(7/30)in the control group,and there was statistical significance between the two groups(P<0.05).After treatment,the scores of overall health,energy,social function,mental health,bodily pain,role-physical,role-emotional,and physical function of the observation group were(63.26±4.49),(74.26±4.55),(83.66±3.92),(81.49±4.26),(74.49±6.39),(73.53±4.19),(84.53±3.68),and(71.38±4.29)points,which were higher than(54.55±3.26),(61.23±4.02),(76.33±2.76),(61.25±3.76),(66.59±4.71),(61.23±4.43),(73.06±2.51),and(52.30±2.77)points in the control group,and there was statistical significance between the two groups(P<0.05).Conclusion Internal fixation with steel plate and screw has good effect and high value in the treatment of patients with traumatic fractures
