摘　　要：目的探讨血清破骨细胞刺激因子1(OSTF1)、netrin-1与老年骨质疏松患者Ⅰ型前胶原氨基端前肽(PⅠNP)、β-胶原特殊序列(β-CTX)、25-羟维生素D的关系。方法选取50例老年骨质疏松患者为研究组,另选取同期体检健康老年人50例为对照组。检测并比较两组外周血清OSTF1、netrin-1和PⅠNP、β-CTX、25-羟维生素D水平,比较不同OSTF1分组(OSTF1≥1.84 ng/ml为OSTF1高组,OSTF1<1.84 ng/ml为OSTF1低组,各25例)、不同netrin-1分组(netrin-1≥1.1 ng/ml为netrin-1高组,netrin-1<1.1 ng/ml为netrin-1低组,各25例)患者的PⅠNP、β-CTX、25-羟维生素D水平。结果研究组患者的OSTF1(1.51±0.23)ng/ml、netrin-1(0.97±0.15)ng/ml、PⅠNP(45.21±5.33)ng/ml和25-羟维生素D(18.50±3.75)ng/ml低于对照组的(2.17±0.34)、(1.23±0.15)、(60.12±4.83)、(25.31±4.22)ng/ml,而β-CTX(0.63±0.09)ng/ml高于对照组的(0.45±0.05)ng/ml(P<0.05)。OSTF1高组的PⅠNP(48.93±3.53)ng/ml、25-羟维生素D(24.51±2.92)ng/ml均显著高于OSTF1低组的(41.49±3.82)、(12.49±2.47)ng/ml,β-CTX(0.55±0.11)ng/ml低于OSTF1低组的(0.71±0.15)ng/ml(P<0.05);netrin-1高组的PⅠNP(46.90±3.48)ng/ml、25-羟维生素D(24.35±2.67)ng/ml显著高于netrin-1低组的(43.52±3.73)、(12.65±2.58)ng/ml(P<0.05);netrin-1高组的β-CTX水平与netrin-1低组比较无显著差异(P>0.05)。结论血清OSTF1、netrin-1与老年骨质疏松患者的PⅠNP、β-CTX、25-羟维生素D等骨质疏松标志物有密切关系,临床可进一步探讨各标志物之间的相互作用机制以预防和治疗骨质疏松。Objective To explore the correlation of serum osteoclast stimulation factor 1(OSTF1)and netrin-1 with typeⅠprocollagen propeptide(PⅠNP)andβ-crosslaps(β-CTX),25-hydroxyvitamin D in elderly patients with osteoporosis.Methods 50 elderly patients with osteoporosis were selected as the study group,and 50 healthy elderly subjects were selected as the control group.The peripheral serum levels of OSTF1,netrin-1 and PⅠNP,β-CTX and 25-hydroxyvitamin D were detected and compared between the two groups,as well as the levels of PⅠNP,β-CTX and 25-hydroxyvitamin D among patients in different OSTF1 subgroups(OSTF1≥1.84 ng/ml for high OSTF1 group,OSTF1<1.84 ng/ml for low OSTF1 group,each with 25 cases)and different netrin-1 subgroups(netrin-1≥1.1 ng/ml for high netrin-1 group,netrin-1<1.1 ng/ml for low netrin-1 group,each with 25 cases).Results The study group had OSTF1 of(1.51±0.23)ng/ml,netrin-1 of(0.97±0.15)ng/ml,PⅠNP of(45.21±5.33)ng/ml and 25-hydroxyvitamin D of(18.50±3.75)ng/ml,which were lower than(2.17±0.34),(1.23±0.15),(60.12±4.83)and(25.31±4.22)ng/ml in the control group;the study group had higherβ-CTX of(0.63±0.09)ng/ml than(0.45±0.05)ng/ml in the control group(P<0.05).High OSTF1 group had PⅠNP of(48.93±3.53)ng/ml and 25-hydroxyvitamin D of(24.51±2.92)ng/ml,which were significantly higher than(41.49±3.82)and(12.49±2.47)ng/ml in low OSTF1 group;high OSTF1 group had lowerβ-CTX of(0.55±0.11)ng/ml than(0.71±0.15)ng/ml in low OSTF1 group(P<0.05).High netrin-1 group had PINP of(46.90±3.48)ng/ml and 25-hydroxyvitamin D of(24.35±2.67)ng/ml,which were significantly higher than(43.52±3.73)and(12.65±2.58)ng/ml in low netrin-1 group(P<0.05).There was no significant difference inβ-CTX level between high netrin-1 group and low netrin-1 group(P>0.05).Conclusion Serum OSTF1 and netrin-1 are closely related to the PⅠNP,β-CTX,25-hydroxyvitamin D and other osteoporosis markers in elderly patients with osteoporosis.The interaction mechanism between these markers can be further explored in clin
