中西医结合治疗慢性肾脏病4期的疗效分析

摘　　要：目的探讨中西医结合治疗慢性肾脏病4期(CKD4期)的疗效。方法120例CKD4期患者,随机分为观察组和对照组,每组60例。对照组使用常规西医治疗,观察组在对照组基础上使用中医治疗。比较两组患者临床疗效、中医证候积分、肾功能指标。结果观察组治疗总有效率95.00%高于对照组的75.00%(P<0.05)。观察组患者腰膝酸软、倦怠乏力、畏寒肢冷、肢体困重评分分别为(0.91±0.15)、(0.86±0.14)、(0.89±0.15)、(0.85±0.13)分,均低于对照组的(1.46±0.23)、(1.40±0.21)、(1.45±0.22)、(1.39±0.21)分(P<0.05)。治疗后,观察组患者尿素氮(BUN)(10.37±3.02)mmol/L、血肌酐(Scr)(264.35±102.39)μmol/L、肾小球过滤率(eGFR)(39.62±9.54)ml/(min·1.73 m^(2))、尿N-乙酰葡萄糖苷酶(NAG酶)/肌酐(1.95±0.57)优于对照组的(14.28±4.31)mmol/L、(335.83±119.68)μmol/L、(34.75±7.98)ml/(min·1.73 m^(2))、(2.74±0.63)(P<0.05)。结论中西医结合治疗CKD4期疗效显著,能有效改善肾功能,缓解证候,抑制肾功能进一步恶化。Objective To explore the therapeutic effect of integrated traditional Chinese and Western medicine on stage 4 chronic kidney disease(CKD4).Methods 120 cases of CKD4 patients were randomly divided into an observation group and a control group,with 60 cases in each group.The control group was treated with conventional Western medicine,and on this basis,the observation group was treated with traditional Chinese medicine on the basis of the control group.The clinical efficacy,traditional Chinese medicine syndrome score and renal function index of the two groups were compared.Results The total effective rate of 95.00%in the observation group was higher than 75.00%in the control group(P<0.05).The scores of lumbar debility,fatigue and lack of strength,and intolerance to cold and cold limbs in the observation group were(0.91±0.15),(0.86±0.14),(0.89±0.15),and(0.85±0.13)points,which were lower than(1.46±0.23),(1.40±0.21),(1.45±0.22),and(1.39±0.21)points in the control group(P<0.05).After treatment,the observation group had blood urea nitrogen(BUN)of(10.37±3.02)mmol/L,serum creatinine(Scr)of(264.35±102.39)μmol/L,estimated glomerular filtration rate(eGFR)of(39.62±9.54)ml/(min·1.73 m^(2)),and urinary N-acecyl-β-D-glucosaminidase(NAG)enzyme/creatinine of(1.95±0.57),which were better than(14.28±4.31)mmol/L,(335.83±119.68)μmol/L,(34.75±7.98)ml/(min·1.73 m^(2)),and(2.74±0.63)in the control group(P<0.05).Conclusion Integrated traditional Chinese and Western medicine has a significant effect on CKD4,which can effectively improve renal function,relieve symptoms and inhibit the further deterioration of renal function.
