中西医结合治疗猫神经源性尿潴留的病例报告

摘　　要：1只6月龄雄性家养短毛猫,从4楼坠楼后出现尿潴留。结合患猫既往病史、临床检查、实验室检查、X线检查和超声检查,西医诊断为创伤性脊髓损伤继发神经源性尿潴留;中医辨证为“痹证”合并“癃闭”范畴,患猫因外伤使督脉受损,气血瘀滞,脾气和肾阳亏虚,膀胱气化失司。西医治疗通过导尿舒缓膀胱压力,用药营养神经组织;中医治疗通过口服中药、激光和针刺。经中西医结合治疗20日后,患猫恢复自主排尿能力。本病例的诊治经验可为类似疾病的中西医结合治疗提供思路。A 6-month-old male domestic shorthair cat developed urinary retention after falling from the fourth floor.Based on the cat's medical history,clinical examination,laboratory tests,radiography,and ultrasonography,the Western medical diagnosis was traumatic spinal cord injury with secondary neurogenic urinary retention.According to Traditional Chinese Medicine(TCM)syndrome differentiation,the case was classified under"Bi Syndrome"with"Long Syndrome"(dysuria and urinary retention),caused by trauma-induced damage to the Du Meridian,leading to stagnation of Qi and Blood,deficiency of Spleen Qi and Kidney Yang,and bladder dysfunction.Western medical treatment included catheterization to relieve bladder pressure and medication to nourish neural tissues,while TCM therapy consisted of oral Chinese herbal medicine,laser therapy,and acupuncture.After 20 days of integrative treatment,the cat regained normal urination.This case provides insights into the combined use of traditional Chinese and Western medicine for similar conditions.
