虚瘀理论下补阳还五汤在老年骨折术后的应用

摘　　要：作为中医骨伤科指导骨折治疗的基本原则,骨折三期辨治原则具体描述为:骨折早期活血化瘀、消肿止痛;中期合营生新、续筋接骨;后期补益肝肾、强壮筋骨。继手术治疗广泛运用后,骨折术后的病理状况已不同于以往三期辨证。文章基于王氏气血理论所衍生的虚瘀理论,对老年骨折术后以气虚血弱和气滞血瘀所致虚实夹杂为特点的气虚血瘀证展开论述。从理法方药4个层面探讨骨折术后的病理状况和防治特点,并突出补阳还五汤加减治疗老年骨折术后诸证的临床效果,开阔骨伤科治疗思路,提高临床疗效。As the basic principle of orthopedics and traumatology department of traditional Chinese medicine to guide the treatment of fracture,the principle of three-stage treatment of fracture is specifically described as promoting blood circulation and removing blood stasis,reducing swelling and relieving pain in the early stage of fracture,mid-term joint production of new,continuous tendon bone,tonifying liver and kidney,strong muscles and bones late.Following the extensive use of surgical treatment in modern medicine,the pathological condition of postoperative fracture,which is different from the previous three-phase diagnosis,has gradually been paid attention to by scholars.Based on the deficiency and stasis theory derived from Wang’s theory of qi and blood,this chapter discussed the“qi deficiency and blood stasis syndrome”,which is a mixture of deficiency and solidity formed by qi deficiency and qi stagnation,and blood stasis,especially in the elderly patients,after major surgery for fracture.The characteristics of the diagnosis were discussed.It discussed the pathological conditions and prevention and treatment characteristics of postoperative fracture from the four levels of rationale,method,prescription and medicine,and highlights the practicality of the addition of Buyang Huanwu decoction in the treatment of postoperative syndromes after major fracture surgery in the elderly,so as to broaden the orthopedics and traumatology treatment ideas and improve the clinical efficacy.
