摘  要:目的:通过对血培养结果和降钙素原(PCT)定量数值的分析,预测PCT浓度水平对血流感染病原菌种的鉴别价值。方法:收集2022年7月—2023年6月本院各科室血培养结果与PCT定量数值进行分析。根据血培养结果分为血培养阳性组和血培养阴性组,血培养阳性组分为革兰阳性菌组和革兰阴性菌组;根据所属科室分为内科组、外科组、肿瘤科组及重症学科组,对血培养菌种与PCT水平进行分析,预测PCT水平测定在鉴别血流感染菌种中的辅助诊断价值。结果:①血培养阳性组PCT中位数水平为3.73(0.36,15.15)μg/L,高于血培养阴性组,差异有统计学意义(P<0.05);革兰阴性菌组PCT中位数水平为7.88(0.92,25.00)μg/L,高于革兰阳性菌组,差异有统计学意义(P<0.05),其余各菌组PCT水平比较,差异均无统计学意义(P>0.05)。②血流感染的各科室间菌种的构成比有差异,其中重症学科组以革兰阳性感染菌占比最大(47.7%),内科组、外科组、肿瘤科组以革兰阴性菌感染为主,尤其外科组占比最大(73.3%);单株感染菌种中:重症学科组的PCT阳性率(87.7%)最高,肿瘤科组的PCT阳性率(61.9%)最低。结论:PCT水平在一定程度上可快速辅助诊断血流感染,结合科室区域流行菌种及其PCT阳性率可提高预判病原菌种的准确度。Objective:Analyze blood culture results and quantitative values of procalcitonin(PCT)to predict the value of PCT concentration levels in identifying pathogenic strains of bloodstream infections.Methods:Blood culture results and quantitative values of PCT were collected and analyzed from July 2022 to June 2023 in each department of our hospital.According to the blood culture results,the subjects were divided into blood culture positive group and blood culture negative group,and the blood culture positive group was divided into Gram-positive bacteria group and Gram-negative bacteria group,and according to the departments they belonged to,they were divided into internal medicine group,surgical group,oncology group and the critical care group,the values of the blood culture strains and PCT levels are analyzed to predict the value of the measurement of the level of PCT as a supplementary diagnostic value in the identification of the strains of the bloodstream infections.Results:①The median level of PCT in the blood culturepositive group was 3.73(0.36,15.15)μg/L,higher than the blood culture-negative group(P<0.05);and the median level of PCT in infected persons in the gram-negative bacteria group was 7.88(0.92,25.00)μg/L,significantly higher than 1.00(0.37,5.96)μg/L in the gram-positive bacteria group,and the difference were statistically significant(P<0.05).The difference in PCT levels of the rest of the bacterial groups was not statistically significant when compared the levels of PCT in the remaining groups(P>0.05).②There were differences in the composition ratio of strains of bloodstream infections among departments,with grampositive infectious organisms accounting for the largest proportion in the intensive care unit group(47.7%),and gram negative infections predominating in the medical,surgical,and oncology groups,especially the largest proportion in the surgical group(73.3%).Among the single infectious organisms,the rate of PCT positivity was the highest in the inten sive care unit group(87.7%),and the ra
