摘 要:目的探讨IgA肾病与“热”证的关联及金水清合剂对IgA肾病大鼠TRPV1水平的影响。方法将54只SD大鼠随机分为正常组、对照组、治疗组,对照组、治疗组予以建立大鼠IgA肾病模型,治疗组在对照组的基础上,给予金水清合剂灌胃后处死后取肾脏组织,采用荧光免疫观察肾小球病理改变,采用Western-blot检测下丘脑及肾组织中TRPV1蛋白的表达。结果与正常组相比,对照组和治疗组的血肌酐(Cr)、尿素氮(BUN)、谷草转氨酶(AST)水平明显升高(P<0.0001),治疗组Cr、BNU、AST浓度低于对照组,有统计学差异(P<0.05;P<0.01;P<0.0001);与正常组相比,对照组与治疗组大鼠肾小球中可见IgA免疫复合物的沉积,其中,对照组IgA免疫复合物相对较多;与正常组相比,对照组与治疗组大鼠下丘脑及肾脏中TRPV1蛋白浓度明显增多,有统计学差异(P<0.01),与对照组相比,治疗组下丘脑及肾脏中TRPV1蛋白浓度有所下降,有统计学差异(P<0.01)。结论IgA肾病大鼠“热”证客观存在,金水清合剂能显著改善IgA肾病大鼠肾脏损害,其作用机制可能与其可下调下丘脑及肾组织中TRPV1蛋白浓度有关。Objective To explore the correlation between IgA nephropathy and heat syndrome,and the effect of Jinshuiqing Mixture(JSQM)on the level of TRPV1 in IgA nephropathy rats.Methods Fifty-four SD rats were randomly divided into the normal group,the control group,and the treatment group.An IgA nephropathy model was established in the control and treatment groups.Based on the control group,the treatment group was gavaged with JSQM and euthanized.Renal tissues were taken to observe the pathological changes of glomeruli by using fluorescence immunoassay.Western blot(WB)was used to detect the expression of TRPV1 protein in the hypothalamus and renal tissues.Results Compared with the normal group,the levels of serum creatinine(Cr),blood urea nitrogen(BUN),and aspartate aminotransferase(AST)in the control and treatment groups were significantly increased(P<0.0001),while the concentrations of Cr,BUN,and AST in the treatment group were lower than those in the control group(P<0.05;P<0.01;P<0.0001).Compared with the normal group,the deposition of IgA immune complexes was observed in the glomeruli of rats in the control and treatment groups.Among them,the control group had relatively more IgA immune complexes.Compared with the normal group,the concentration of TRPV1 protein in the hypothalamus and kidneys of the control and treatment groups rats was significantly increased(P<0.01).Compared with the control group,the concentration of TRPV1 protein in the hypothalamus and kidneys of the treatment group was decreased(P<0.01).Conclusion The heat syndrome objectively exists in IgA nephropathy rats,and JSQM can significantly improve renal damage in IgA nephropathy rats whose mechanism of action may be related to its ability to down-regulate the concentration of TRPV1 protein in the hypothalamus and renal tissue.
