2020-2023年北京市大兴区不同人群碘营养水平监测结果分析  

Analysis of monitoring results of iodine nutrition levels in different populations in Daxing District,Beijing from 2020 to 2023

摘  要:目的了解大兴区不同人群的碘营养水平,为"因地制宜、分类指导、科学补碘"的碘缺乏病防治措施实施提供依据。方法2020—2023年,每年4—9月将北京市大兴区按东、西、南、北、中划分为5个片区,每个片区抽取1个乡镇(街道),每个乡镇(街道)抽取孕妇20名、育龄妇女和成年男性各40名,以及1所小学的8~10岁非寄宿儿童40名;同时,每年在大兴区选择1所产科医院抽取现住址为北京市大兴区的孕妇100名。采集所有调查对象的1次随机尿样,儿童及400名孕妇的家中食用盐盐样,进行尿碘及盐碘含量测定。结果2020—2023年,共采集尿样3022份,其中8~10岁儿童、孕妇、育龄妇女和成年男性分别为920、700、702、700份,尿碘中位数依次为181.1、135.0、144.0、140.0μg/L。不同年份8~10岁儿童、孕妇、育龄妇女尿碘水平比较,差异均有统计学意义(H=77.88、9.40、22.11,均P<0.05)。共采集8~10岁儿童及孕妇家庭食用盐盐样1320份,其中儿童盐样920份,盐碘中位数为21.3 mg/kg,碘盐覆盖率为88.48%(814/920),合格碘盐食用率为80.76%(743/920);孕妇盐样400份,盐碘中位数为21.8 mg/kg,碘盐覆盖率为90.25%(361/400),合格碘盐食用率为77.50%(310/400)。相关性分析结果显示,8~10岁儿童、孕妇尿碘含量与其家庭食用盐碘含量均无相关性(r=0.06、0.07,P=0.055、0.142)。结论北京市大兴区孕妇碘营养处于缺乏水平,8~10岁儿童、育龄妇女和成年男性均处于碘适宜水平。应加强对孕妇人群的健康教育,保证孕妇充足的碘营养。同时,有关部门应加大碘盐监督管理力度。Objective To investigate the iodine nutrition level of different populations in Daxing District of Beijing,and to provide a basis for implementation of"tailored measures,classified guidance,and scientific iodine supplementation"for prevention and control of iodine deficiency disorders.Methods From 2020 to 2023,Daxing District of Beijing was divided into 5 districts from April to September each year based on east,west,south,north,and center.One township(street)was selected from each district,and 20 pregnant women,40 women of childbearing age,40 adult males,and 40 non boarding children aged 8 to 10 from one primary school were selected from each township(street).At the same time,100 pregnant women with current addresses in Daxing District,Beijing were selected from one obstetrics hospital in Daxing District every year.Once random urine sample was collected from all survey subjects,and the household edible salt samples were collected from children and 400 pregnant women for determination of urinary iodine and salt iodine levels.Results From 2020 to 2023,a total of 3022 urine samples were collected,including 920,700,702,and 700 samples from children aged 8-10,pregnant women,women of childbearing age,and adult males,respectively.The median urinary iodine levels were 181.1,135.0,144.0,and 140.0μg/L,respectively.There were statistically significant differences in urinary iodine levels among children aged 8-10,pregnant women,and women of childbearing age in different years(H=77.88,9.40,22.11,P<0.05).A total of 1320 household edible salt samples were collected from children aged 8-10 and pregnant women.Among them,920 salt samples were collected from children,the median salt iodine was 21.3 mg/kg,the iodized salt coverage rate was 88.48%(814/920),and the qualified iodized salt consumption rate was 80.76%(743/920).Four hundred salt samples were collected from pregnant women,the median salt iodine was 21.8 mg/kg,the coverage rate of iodized salt was 90.25%(361/400),and the consumption rate of qualified iodized salt was 77.5
