腰椎后路手术脑脊液漏危险因素分析及护理干预

摘　　要：目的探讨腰椎后路手术中脑脊液漏的危险因素及护理措施。方法对330例住院行腰椎后路手术患者的临床资料进行回顾性分析。根据术后脑脊液漏的发生情况将患者分为脑脊液漏组(n=22)和无脑脊液漏组(n=308)。分析后路腰椎手术发生脑脊液漏的危险因素,并评价护理干预措施。结果330例后路腰椎手术患者中,脑脊液漏发生率为6.7%。单因素分析显示年龄、吸烟史、饮酒史、手术类型、减压节段、硬脑膜粘连是腰椎后路手术脑脊液漏的危险因素。年龄、硬脑膜粘连、翻修手术是腰椎术后脑脊液漏的独立危险因素(P<0.05)。结论根据腰椎后路手术后脑脊液漏的危险因素分析,年龄、硬脑膜粘连、翻修手术增加术后脑脊液漏的危险。Objective To explore the risk factors and nursing measures of cerebrospinal fluid leakage in posterior lumbar surgery.Methods A retrospective analysis was conducted on clinical data of 330 hospitalized patients who underwent posterior lumbar surgery.According to the incidence of postoperative cerebrospinal fluid leakage,the patients were divided into a cerebrospinal fluid leakage group(n=22)and a non-cerebrospinal fluid leakage group(n=308).The risk factors of cerebrospinal fluid leakage in posterior lumbar surgery were analyzed and the nursing interventions were evaluated.Results Among the 330 patients who underwent posterior lumbar surgery,the incidence of cerebrospinal fluid leakage was 6.7%.Univariate analysis revealed that age,smoking history,alcohol consumption history,surgical type,decompression segment,and dural adhesion were risk factors for cerebrospinal fluid leakage in posterior lumbar spine surgery.Age,dural adhesion,and revision surgery were identified as independent risk factors for postoperative cerebrospinal fluid leakage in lumbar spine surgery(P<0.05).Conclusions According to the analysis of risk factors for cerebrospinal fluid leakage after posterior lumbar surgery,age,dural adhesion,and revision surgery increased the risk of postoperative cerebrospinal fluid leakage.
