基于《黄帝内经》“督脉主痔”和“阳化气,阴成形”理论论治痔病

摘　　要：痔病是最常见的肛周良性疾病,发病率高。文章基于《黄帝内经》“督脉主痔”和“阳化气,阴成形”相关理论探讨痔病发病机理,从经络学说剖析“督脉主痔”之内涵包括督绕篡后、生病主痔,督脉络肾、血流沉痔和阳不柔筋、筋脉横解;而肾督主阳,“阳化气,阴成形”,痔病之源乃肾督之阳不能化气,浊阴凝滞,魄门瘀毒阻遏。据此提出“通补肾督”与“化气散形”论治痔病之思路,具体治法包括温养肾督、化气散形,达郁通阳、决流络血和安位相火、风毒自弭三方面。Hemorrhoids is the most common perianal benign disease with a high incidence.In this paper,based on the theory of Yellow Emperor's Inner Canon,‘Governor Vessel governing hemorrhoids’and‘Yang transforming Qi,Yin forming’,the pathogenesis of hemorrhoids is discussed.The connotation of‘Governor Vessel governing hemorrhoids’is analyzed from the theory of meridians and collaterals,including Governor Vessel surrounding usurping,disease governing hemorrhoids,Governor Vessel kidney,blood flow sinking hemorrhoids and Yang not softening tendons,tendons and veins transverse solution;the kidney governor governs yang,‘yang transforming qi,yin forming’.The source of hemorrhoids is that the yang of the kidney governor cannot transform qi,turbidity and yin are stagnated,and the blood stasis and toxin of the gate are blocked.Based on this,the idea of‘dredging kidney and Du meridian’and‘resolving qi and dispersing shape’is proposed to treat hemorrhoids.The specific treatment methods include warming kidney and Du meridian,resolving qi and dispersing shape,relieving depression and dredging yang,resolving blood stasis and collaterals,calming phase fire,and self-eliminating wind and toxin.
