温胞饮加减联合隔姜灸脐法对反复胚胎种植失败患者子宫内膜容受性及妊娠结局的影响

摘　　要：目的探究反复胚胎种植失败(RIF)患者应用温胞饮加减联合隔姜灸脐法治疗的临床效果。方法选取2023年1月至2024年1月于江西省妇幼保健院生殖中心助孕的100例RIF拟再次行替代周期冻融胚胎移植(FET)肾阳虚患者作为研究对象,采用随机数字表法分为治疗组(50例)及对照组(50例)。对照组采用FET激素替代周期治疗,治疗组在对照组基础上行温胞饮加减联合隔姜灸脐疗治疗,连续治疗3个月。比较两组中医症状积分、子宫内膜容受性、妊娠情况及不良反应。结果治疗组治疗后主症及次症评分均低于对照组,差异有统计学意义(P<0.05)。两组治疗后子宫内膜形态及子宫内膜下血流灌注比较,差异无统计学意义(P>0.05);治疗组治疗后子宫内膜厚度大于对照组,阻力指数(RI)及搏动指数(PI)均低于对照组,差异有统计学意义(P<0.05)。治疗组的临床妊娠率高于对照组,差异有统计学意义(P<0.05);两组胚胎种植率比较,差异无统计学意义(P>0.05)。两组治疗期间均未见明显不良反应。结论RIF患者应用温胞饮加减联合隔姜灸脐法治疗能够有效改善患者各临床症状,同时还可改善子宫内膜厚度、血流参数,在一定程度上改善患者子宫内膜容受性,对提升临床妊娠率具有重要作用,值得推广应用。Objective To explore the clinical effect of Modified Wenbao Yin combined with ginger-insulated moxibustion on umbilicus treatment on patients with repeated implantation failure(RIF).Methods A total of 100 patients with kidney yang deficiency undergoing alternative cycle frozen thawed embryo transfer(FET)in RIF who were pregnant in the reproductive center of Jiangxi Maternal and Child Health Hospital from January 2023 to January 2024 were selected as the research objects,and they were divided into treatment group(50 cases)and control group(50 cases)according to random number table method.The control group was treated with FET hormone replacement cycle therapy,and the treatment group was treated on the basis of the control group with Modified Wenbao Yin combined with ginger-insulated moxibustion therapy,continuous treatment for 3 months.Traditional Chinese medicine symptom score,endometrial receptivity,pregnancy status and adverse reactions were compared between the two groups.Results The scores of the main and secondary syndromes in the treatment group were lower than those in the control group after treatment,and the differences were statistically significant(P<0.05).There were no statistically significant differences in endometrial morphology and subendometrial blood flow perfusion between the two groups after treatment(P>0.05).After treatment,the endometrial thickness of the treatment group was greater than that of the control group,and the resistance index(RI)and pulsatility index(PI)were lower than those of the control group,with statistical significances(P<0.05).The clinical pregnancy rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).The difference in embryo implantation rate between the two groups was not statistically significant(P>0.05).No significant adverse reactions were observed during the treatment period in both groups.Conclusion The application of Modified Wenbao Yin combined with ginger-insulated moxibustion on umbilicus
