摘 要:卵巢早衰(POF)是一种严重影响育龄期女性生殖健康的内分泌疾病。近年来,随着再生医学的发展,间充质干细胞治疗POF的方法受到广泛关注。脂肪间充质干细胞(ADMSC)因其易于获取、低免疫原性及多向分化潜能等优势,在POF治疗中展现出良好的应用前景。ADMSC通过多重作用机制参与卵巢功能的修复,即ADMSC能分化为颗粒细胞样细胞,直接参与卵泡发育过程;ADMSC可通过旁分泌途径分泌多种生长因子和细胞因子调节卵巢微环境,促进血管新生,改善局部血供;ADMSC具有显著的抗炎和免疫调节作用,可降低炎症因子水平,减轻氧化应激损伤,促进卵巢结构及储备功能的恢复。然而,ADMSC治疗POF仍面临细胞存活率低、归巢效率不理想等挑战。本文综述ADMSC在POF治疗中的作用机制及研究进展,为后续的临床治疗和研究提供理论依据。Premature ovarian failure(POF)is an endocrine disease that seriously affects reproductive health of women of childbearing age.In recent years,with the development of regenerative medicine,the method of mesenchymal stem cell in the treatment of POF has received widespread attention.Adipose derived mesenchymal stem cell(ADMSC)has shown promising application prospects in POF therapy due to their advantages of easy accessibility,low immunogenicity,and multipotent differentiation potential.ADMSC participates in the repair of ovarian function through multiple mechanisms of action,that is,ADMSC can differentiate into granulosa like cells,directly participate in the follicular development process;ADMSC can secrete various growth factors and cytokines through paracrine pathways to regulate ovarian microenvironment,promote angiogenesis,improve local blood supply;ADMSC has significant anti-inflammatory and immune regulatory effects,which can reduce levels of inflammatory factors,alleviate oxidative stress damage,promote the recovery of ovarian structure and reserve function.However,ADMSC in the treatment of POF still faces challenges such as low cell survival rate and unsatisfactory homing efficiency.This article reviews action mechanism and research progress of ADMSC in the treatment of POF,providing theoretical basis for subsequent clinical treatment and research.
