摘　　要：中心动脉(主动脉)血压是主要脏器的血液灌注压力,可能与靶器官损害有密切关系,有心血管事件的预测价值,外周动脉(肱动脉)血压不能代替中心动脉血压。中心动脉血压检测技术已日趋成熟且实现自动化,近年来采用示波血压测量技术的动态血压计可同步评估中心动脉血压和外周动脉血压,两者差异的相关研究越来越受到重视。基于两者差异出现3个高血压新表型:孤立性外周动脉高血压、孤立性中心动脉高血压、中心动脉和外周动脉一致性高血压。但这3个新表型的诊断及应用价值尚存许多争议。本文对中心动脉血压的形成原理、测量方法及在高血压分型中的临床应用进行综述。Central arterial(aorta)blood pressure is blood perfusion pressure of major organs,which may be closely related to target organ damage,has predictive value for cardiovascular events.Peripheral arterial(brachial artery)blood pressure cannot replace central artery blood pressure.Detection technology of central arterial blood pressure has become increasingly mature and automated.In recent years,dynamic blood pressure monitors using oscilloscope blood pressure measurement technology can simultaneously evaluate central arterial blood pressure and peripheral arterial blood pressure,the related research on the differences between the two is receiving increasing attention.Based on the differences between the two,three new hypertension phenotypes have emerged:isolated peripheral arterial hypertension,isolated central arterial hypertension,central arterial and peripheral arterial consistent hypertension.However,there is still much controversy over diagnostic and practical value of these three new phenotypes.This article reviews formation principles,measurement methods of central arterial blood pressure,clinical application in hypertension classification.
