经输尿管镜钬激光术治疗尿道狭窄合并尿道结石的临床疗效研究

摘　　要：目的分析经输尿管镜钬激光术治疗尿道狭窄合并尿道结石的临床疗效。方法选择的研究对象是同时合并尿道结石与尿道狭窄的患者,共选择90例,以此次研究对象随机分组,两组中的数量一致,均是45例。对照组患者接受电切镜冷刀术治疗,研究组患者接受经输尿管镜钬激光术治疗。比较两组患者临床疗效、一般手术指标、并发症发生情况及疼痛评分[视觉模拟评分法(VAS)评分]、生活质量量表(QOL)评分。结果从碎石结果上来看,研究组上段的成功率与对照组对比有显著性差异(P<0.05),但两组的中段以及下段结石的碎石结果对比并无显著性差异(P>0.05)。研究组上段结石清除率高于对照组(100.0%VS 44.4%),差异具有统计学意义(P<0.05);经比较,中段与下段的清除率对比无显著差异(P>0.05)。从各项手术相关指标的对比结果上来看,研究组的各项指标均是优于对照组(P<0.05)。术后1 d,两组VAS评分、QOL评分均低于入院时,且研究组VAS评分(2.6±0.6)分、QOL评分(1.8±0.3)分低于对照组的(3.5±0.7)、(2.3±0.3)分,差异具有统计学意义(P<0.05)。从并发症的对比上来看,研究组的优势更为明显,发生率更低(P<0.05)。结论对尿路狭窄合并尿道结石患者开展经输尿管镜钬激光术治疗效果肯定,有助于加速患者术后康复进程,缓解患者术后疼痛度,提高患者生活质量,且安全性较高。Objective To analyze the clinical efficacy of transurethral holmium laser in the treatment of urethral stricture combined with urethral calculi.Methods 90 patients diagnosed with urethral stricture combined with urethral calculi were selected as the research subjects.According to the random number table method,they were divided into the study group and the control group,each consisting of 45 patients.The control group received resectoscope cold knife surgery,and the study group received transurethral holmium laser surgery.The clinical efficacy,general surgical indicators,complications,pain score[Visual Analogue Scale(VAS)score],and quality of life(QOL)score were compared between the two groups.Results From the results of lithotripsy,there was a significant difference in the success rate of the upper segment between the study group and the control group(P<0.05),but there was no significant difference in the lithotripsy results of the middle and lower segments between the two groups(P>0.05).The stone clearance rate of the upper segment in the study group was higher than that in the control group(100.0%VS 44.4%),and the difference was statistically significant(P<0.05).After comparison,there was no significant difference in stone clearance rate of the middle segment and the lower segment(P>0.05).From the comparison results of various operation-related indicators,all indicators of the study group were better than those of the control group(P<0.05).1 d after surgery,VAS score and QOL score in both groups were lower than those at admission;the study group had VAS score of(2.6±0.6)points and QOL score of(1.8±0.3)points,which were lower than(3.5±0.7)and(2.3±0.3)points in the control group;the differences were statistically significant(P<0.05).From the comparison of complications,the advantage of the study group was more obvious,and the incidence was lower(P<0.05).Conclusion Transurethral holmium laser is effective in the treatment of urinary tract stricture combined with urethral calculi,which is helpful to accelerate
