血糖检验和尿糖检验在糖尿病诊断中的意义讨论

摘　　要：目的分析血糖检验和尿糖检验在糖尿病诊断中的应用价值。方法选取65例疑似糖尿病患者作为研究对象,为患者实施血糖检验和尿糖检验。对比血糖检验和尿糖检验的阳性检出率;将综合诊断结果作为金标准,对比血糖检验和尿糖检验对糖尿病的诊断效能;分析血糖检验和尿糖检验与金标准的一致性。结果65例患者经金标准确诊糖尿病60例,确诊率为92.31%。血糖检验检出阳性59例,检出率为90.77%;尿糖检验检出阳性54例,检出率为83.08%。血糖检验的阳性检出率高于尿糖检验,但差异无统计学意义(P>0.05)。血糖检验对糖尿病的诊断灵敏度96.67%、准确度95.38%、阴性预测值66.67%均高于尿糖检验的85.00%、81.54%、18.18%(P<0.05)。血糖检验结果与金标准的Kappa值为0.84(基本完全一致),尿糖检验结果与金标准的Kappa值为0.57(一致性中等)。结论血糖检验对糖尿病的诊断效能较尿糖检验高,因此在临床糖尿病诊断中可以先为患者实施血糖检验,若血糖检验结果无法作为参考则可配合尿糖检验,进一步提高糖尿病的确诊率,为患者尽快得到有效的治疗奠定基础。Objective To analyze the application value of blood glucose test and urine glucose test in the diagnosis of diabetes.Methods 65 patients with suspected diabetes were selected as research subjects,and blood glucose test and urine glucose test were carried out for patients.Comparison of the positive detection rates of the blood glucose test and the urine glucose test;comparison of the diagnostic efficacy of the blood glucose test and the urine glucose test for diabetes using the combined diagnostic results as the gold standard;analysis of the consistency of the blood glucose test and the urine glucose test with the gold standard.Results Among the 65 patients,60 cases had diabetes by the gold standard,with a detection rate of 92.31%.Blood glucose test showed 59 positive cases,with a detection rate of 90.77%;urine glucose test showed 54 positive cases,with a detection rate of 83.08%.The positive rate of blood glucose test was higher than that of urine glucose test,but the difference was not statistically significant(P>0.05).The diagnostic sensitivity(96.67%),accuracy(95.38%)and negative predictive value(66.67%)of blood glucose test were higher than those of urine glucose test(85.00%,81.54%and 18.18%)(P<0.05).The Kappa value between blood glucose test against the gold standard was 0.84(basic consistency),and the Kappa value between urine glucose test against the gold standard was 0.57(moderate consistency).Conclusion The diagnostic efficacy of blood glucose test is higher than that of the urine glucose test.Therefore,blood glucose test can be carried out for patients first in the clinical diagnosis of diabetes.If the blood glucose test results cannot be used as a reference,the urine glucose test can be used to further improve the diagnostic accuracy of diabetes and lay the foundation for patients to get effective treatment as soon as possible.
