黄连汤联合腹针治疗2型糖尿病胃轻瘫的临床疗效观察

摘　　要：目的观察黄连汤联合腹针治疗2型糖尿病胃轻瘫(DGP)的效果。方法86例2型DGP患者,按就诊顺序随机分为对照组和治疗组,各43例。对照组使用枸橼酸莫沙必利片治疗,治疗组在对照组基础上予黄连汤联合腹针治疗。比较两组治疗效果、血糖指标、胃肠功能指标和不良反应发生率。结果治疗组、对照组的总有效率分别是97.67%、83.72%,比较有统计学差异(P<0.05)。治疗后,两组患者的空腹血糖、餐后2 h血糖、糖化血红蛋白水平较治疗前明显降低,且治疗组的空腹血糖(5.68±1.45)mmol/L、餐后2 h血糖(7.20±1.59)mmol/L、糖化血红蛋白(4.48±1.13)%低于对照组的(6.43±1.76)mmol/L、(8.04±1.80)mmol/L、(5.23±1.27)%,有统计学差异(P<0.05)。治疗后,两组患者的胃泌素和胃排空率较治疗前增高,胃排空时间较治疗前缩短,且治疗组的胃泌素(77.60±9.42)ng/L和胃排空率(70.38±8.11)%高于对照组的(55.49±7.60)ng/L、(65.92±6.48)%,胃排空时间(4.63±1.30)h短于对照组的(5.32±1.57)h,有统计学差异(P<0.05)。治疗组不良反应发生率13.95%稍高于对照组的9.30%,但差异无统计学意义(P>0.05)。结论黄连汤联合腹针治疗2型DGP的疗效确切,能进一步控制血糖水平,促进胃肠功能恢复,且治疗安全性较高。Objective To observe the effect of Huanglian Decoction combined with abdominal acupuncture in treating type 2 diabetic gastroparesis(DGP).Methods 86 patients with type 2 DGP were randomly divided into a control group and a treatment group according to the order of their visits,with 43 patients in each group.The control group was treated with mosapride citrate tablets,while the treatment group was treated with Huanglian Decoction combined with abdominal acupuncture on the basis of the control group.Comparison of the treatment effects,blood glucose indicators,gastrointestinal function indicators,and adverse reactions between two groups.Results The total effective rate of the treatment group and the control group were 97.67%and 83.72%,and there was statistical difference(P<0.05).After treatment,the levels of fasting blood glucose,2 h postprandial blood glucose and glycated hemoglobin in both groups were significantly lower than those before treatment;in the treatment group,the fasting blood glucose was(5.68±1.45)mmol/L,the 2 h postprandial blood glucose was(7.20±1.59)mmol/L,and the glycated hemoglobin was(4.48±1.13)%,which were lower than(6.43±1.76)mmol/L,(8.04±1.80)mmol/L,and(5.23±1.27)%in the control group;there was statistical difference(P<0.05).After treatment,the gastrin and gastric emptying rate in both groups were higher than those before treatment,and the gastric emptying time was shorter than that before treatment;the treatment group had gastrin of(77.60±9.42)ng/L and gastric emptying rate of(70.38±8.11)%,which were higher than(55.49±7.60)ng/L and(65.92±6.48)%in the control group;the treatment group had shorter gastric emptying time of(4.63±1.30)h than(5.32±1.57)h in the control group;there was statistical difference(P<0.05).The incidence of adverse reactions in the treatment group(13.95%)was slightly higher than that in the control group(9.30%),but the difference was not statistically significant(P>0.05).Conclusion The combination of Huanglian Decoction and abdominal acupuncture has a definite
