益肾活血降浊汤灌肠治疗慢性肾衰竭脾虚湿浊证的效果分析

摘　　要：目的分析慢性肾功能衰竭(慢性肾衰竭,CRF)脾虚湿浊证患者接受中药益肾活血降浊汤灌肠治疗的效果。方法以数字双盲法将120例CRF脾虚湿浊证患者分为对照组及研究组,对照组60例患者给予西医治疗,研究组60例患者在对照组基础上给予益肾活血降浊汤灌肠治疗。比较两组治疗效果、肾功能、代谢指标及中医证候积分。结果研究组治疗总有效率91.67%较对照组的76.67%高(P<0.05)。治疗后,研究组主症中医证候积分(8.66±1.17)分、次症中医证候积分(5.54±1.00)分低于对照组的(11.03±1.25)、(9.25±1.03)分(P<0.05)。治疗后,研究组尿素氮(BUN)(15.32±1.17)mmol/L、血肌酐(Scr)(323.65±70.06)μmol/L、内生肌酐清除率(Ccr)(365.85±16.73)ml/(min·1.73 m~2)、24 h尿蛋白定量(24 h UTP)(1402.62±76.03)mg/24 h均低于对照组的(18.20±2.14)mmol/L、(365.88±71.87)μmol/L、(395.67±18.66)ml/(min·1.73 m~2)、(1582.24±76.68)mg/24 h(P<0.05)。治疗后,研究组磷(1.12±0.30)mmol/L、甲状旁腺激素(PTH)(112.20±20.18)ng/L低于对照组的(1.35±0.38)mmol/L、(150.18±23.67)ng/L,钙(2.50±0.35)mmol/L高于对照组的(2.23±0.26)mmol/L(P<0.05)。结论CRF脾虚湿浊证采用益肾活血降浊汤进行灌肠治疗,能改善中医证候,调节肾功能及钙磷代谢指标,获得较高的疗效。Objective To analyze the effect of enema with Yishen Huoxue Jiangzhuo Decoction in treating chronic renal failure(CRF)with syndrome of spleen deficiency and dampness turbidity.Methods A total of 120 cases of CRF patients with syndrome of spleen deficiency and dampness turbidity were divided into a control group and a study group by digital double-blind method.60 patients in the control group were given Western medicine treatment,and 60 patients in the study group were given enema with Yishen Huoxue Jiangzhuo Decoction based on the control group.The therapeutic effect,renal function,metabolic indexes and traditional Chinese medicine syndrome score of the two groups were compared.Results The total effective rate of 91.67%in the study group was higher than 76.67%in the control group(P<0.05).After treatment,the traditional Chinese medicine syndrome score of the study group was(8.66±1.17)points for the main symptom and(5.54±1.00)points for the minor symptom,which were lower than(11.03±1.25)and(9.25±1.03)points of the control group(P<0.05).After treatment,the blood urea nitrogen(BUN)in the study group was(15.32±1.17)mmol/L,the serum creatinine(Scr)was(323.65±70.06)μmol/L,the creatinine clearance rate(Ccr)was(365.85±16.73)ml/(min·1.73 m2),and the 24 h urinary total protein(UTP)(24 h UTP)was(1402.62±76.03)mg/24 h,which were lower than(18.20±2.14)mmol/L,(365.88±71.87)μmol/L,(395.67±18.66)ml/(min·1.73 m2),and(1582.24±76.68)mg/24 h in the control group(P<0.05).After treatment,the study group had phosphorus of(1.12±0.30)mmol/L and parathyroid hormone(PTH)of(112.20±20.18)ng/L,which were lower than(1.35±0.38)mmol/L and(150.18±23.67)ng/L in the control group;the study group had higher calcium of(2.50±0.35)mmol/L than(2.23±0.26)mmol/L in the control group(P<0.05).Conclusion Enema with Yishen Huoxue Jiangzhuo Decoction for the treatment of CRF with syndrome of spleen deficiency and dampness turbidity can improve traditional Chinese medicine syndrome,regulate renal function and calcium and calcium and phospho
