摘  要:目的观察全胸震荡排痰系统联合体外膈肌起搏器(EDP)在重症加强护理病房(ICU)脱机困难肺康复患者中的应用效果。方法150例呼吸衰竭患者,所有患者按数字随机分组法分为实验组(80例)和对照组(70例)。对照组进行常规的药物治疗及心肺物理康复治疗,实验组在对照组的治疗基础上联合应用全胸震荡排痰系统及EDP进行治疗。比较两组患者治疗后膈肌厚度、移动度及呼吸机使用时间和住ICU时间。结果治疗后,实验组患者膈肌移动度(3.96±0.72)cm、膈肌厚度(0.29±0.23)cm均大于对照组的(3.02±1.01)、(0.21±0.15)cm,差异显著(P<0.05)。实验组治疗后呼吸机使用时间(213.7±2.1)h、住ICU时间(15.34±2.76)d均短于对照组的(376.5±1.9)h、(21.63±3.15)d,差异显著(P<0.05)。结论全胸震荡排痰系统联合EDP应用于ICU脱机困难肺康复患者中可起到极佳得排痰效果,同时增加其膈肌肌力和耐力,使患者通气功能得到改善,从而使呼吸机使用时间、住ICU时间大大缩短。Objective To observe the application effect of chest wall oscillation expectoration system combined with external diaphragm pacemaker(EDP)in pulmonary rehabilitation of patients experiencing weaning difficulties in intensive care unit(ICU).Methods 150 patients with respiratory failure were randomly divided into an experimental group(80 cases)and a control group(70 cases).The control group was treated with conventional drug therapy and cardiopulmonary physical rehabilitation,and the experimental group was treated with chest wall oscillation expectoration system combined with EDP based on the treatment of the control group.The thickness and mobility of the diaphragm,the duration of ventilator use and length of ICU stay after treatment were compared between the two groups.Results After treatment,the thickness and mobility of the diaphragm of the experimental group were(3.96±0.72)and(0.29±0.23)cm,which were greater than(3.02±1.01)and(0.21±0.15)cm of the control group,and the differences were significant(P<0.05).After treatment,the duration of ventilator use in the experimental group was(213.7±2.1)h and the length of ICU stay was(15.34±2.76)d,which were shorter than(376.5±1.9)h and(21.63±3.15)d in the control group,and the differences were significant(P<0.05).Conclusion Application of chest wall oscillation expectoration system combined with EDP in pulmonary rehabilitation ICU of patients experiencing weaning difficulties in ICU can provide excellent sputum expectoration effect,while increasing their diaphragm muscle strength and endurance,improving their ventilation function,thus greatly reducing the duration of ventilator use and ICU stay.
