不同营养治疗方案在重度颅脑损伤开颅手术后鼻胃管饮食患者中的应用对比

摘  要:目的对比不同营养治疗方案在重度颅脑损伤(SBI)开颅手术后鼻胃管饮食患者中的应用效果。方法回顾性采集2020年2月至2021年7月在郑州颐和医院接受普食流质治疗方案的SBI开颅手术后鼻胃管饮食的40例患者资料,纳入对照组;采集2021年8月至2022年12月医院接受立适康短肽型肠内营养(EN)支持的SBI开颅手术后鼻胃管饮食的40例患者资料,纳入观察组。两组患者均治疗15 d,患者病历资料、治疗相关资料、治疗结果相关资料均保存完整。查阅患者资料,记录患者治疗前后营养状态[前白蛋白(PAB)、白蛋白(ALB)及血红蛋白(Hb)水平];记录患者达到目标营养时间、入住ICU时间及住院总时间;记录患者营养治疗期间相关并发症发生情况。结果治疗前,两组患者各项营养指标水平差异无统计学意义(P>0.05),治疗后,两组患者PAB、ALB、Hb水平升高,且观察组高于对照组(P<0.05);观察组达到目标营养时间、入住ICU时间及住院总时间均较对照组短(P<0.05);治疗期间,观察组营养治疗相关并发症发生率较对照低(P<0.05)。结论立适康短肽型肠内营养治疗,可以促进SBI开颅手术后鼻胃管饮食患者的营养状态改善,促进患者身体恢复、缩短住院治疗时间,减少相关并发症的发生,更具应用价值。Objective To compare the effect of different nutritional treatment schemes in patients with nasogastric tube diet after craniotomy for severe-brain-injury(SBI).Methods A retrospective study was conducted to collect the data of 40 patients who received nasogastric tube diet after SBI craniotomy in Zhengzhou Yihe Hospital from February 2020 to July 2021,and were included in the control group.The data of 40 patients who received nasogastric tube diet after SBI craniotomy supported by Lishikang short peptide enteral nutrition(EN)from August 2021 to December 2022 were collected and included in the observation group.Both groups of patients were treated for 15 days,and the patient’s medical records,treatment related data,and treatment outcome related data were all kept intact.The nutritional status[prealbumin(PAB),albumin(ALB)and hemoglobin(Hb)levels]of patients before and after treatment were recorded.The time of reaching the target nutrition,the time of staying in ICU and the total time of hospitalization were recorded.The occurrence of related complications during nutritional therapy was recorded.Results Before treatment,there was no difference in the levels of various nutritional indicators between the two groups of patients(P>0.05).After treatment,the levels of PAB,ALB,and Hb in the two groups of patients increased,and the observation group was higher than the control group(P<0.05).The observation group had shorter time to achieve target nutrition,stay in ICU,and total hospitalization than the control group(P<0.05).During the treatment period,the incidence of nutritional treatment related complications in the observation group was lower than that in the control group(P<0.05).Conclusion Lishikang short peptide enteral nutrition therapy can promote the improvement of nutritional status of patients with nasogastric tube diet after SBI craniotomy,promote the recovery of patients,shorten the hospitalization time,reduce the occurrence of related complications,and has more application value.
