改良射频消融联合泡沫硬化剂注射治疗下肢静脉曲张的并发症及随访结果

摘　　要：目的分析改良射频消融联合泡沫硬化剂注射治疗下肢静脉曲张的并发症及随访结果。方法选取2020年1月至2022年12月河南中医药大学第一附属医院收治的100例下肢静脉曲张患者,随机数字表法分为对照组和观察组,各50例,对照组接受传统射频消融联合泡沫硬化剂注射治疗,观察组接受改良射频消融联合泡沫硬化剂注射治疗,比较两组手术相关指标、视觉模拟评分法(VAS)评分、并发症、静脉临床严重程度(VCSS)评分、慢性静脉功能不全生活质量问卷(CIVIQ)评分及术后6个月复发率。结果观察组手术时间短于对照组,住院费用少于对照组(P<0.05)。术后2、6、12 h,观察组VAS评分低于对照组(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。术后3 d,观察组VCSS评分低于对照组(P<0.05)。术后6个月,观察组CIVIQ评分高于对照组(P<0.05)。观察组术后6个月的复发率低于对照组(2.00%比14.00%)(P<0.05)。结论改良射频消融联合泡沫硬化剂注射治疗下肢静脉曲张能够缩短手术时间,减少住院费用,缓解疼痛程度,降低复发率,提高生活质量,值得临床推广应用。Objective To compare the complications and follow-up results of modified radiofrequency ablation combined with foam sclerotherapy for lower limb varicose veins.Methods From January 2020 to December 2022,100 patients with lower limb varicose veins admitted to the First Affiliated Hospital of Henan University of Chinese Medicine were randomly divided into control group and observation group,with 50 cases each group.The control group was treated with traditional radiofrequency ablation combined with foam hardener injection,and the observation group was treated with improved radiofrequency ablation combined with foam hardener injection.The two groups were compared with each other in terms of operative time,intraoperative blood loss,length of hospital stay,hospitalization cost,visual analogue scale(VAS)score,complications,venous clinical severity score(VCSS)score,chronic venous insufficiency questionnaire(CIVIQ)score and recurrence rate 6 months after surgery.Results The operation time and hospitalization cost of the observation group were shorter than control group,and the VAS scores at 2,6 and 12 h after operation were lower than control group(P<0.05).There was no difference in complication rate between the two groups(P>0.05).The VCSS score of the observation group was lower than control group at 3 days after surgery,and the CIVIQ score at 6 months after surgery was higher than control group(P<0.05).The recurrence rate 6 months after operation in the observation group was lower control group(2.00%vs 14.00%)(P<0.05).Conclusion Modified radiofrequency ablation combined with foam sclerotherapy for lower limb varicose veins can shorten the operation time,reduce hospitalization costs,relieve pain,reduce recurrence rate and improve quality of life,which is worthy of clinical application.
