丹参川芎嗪注射液对急性胰腺炎患者血清脂肪酶、淀粉酶水平的影响

摘　　要：目的回顾性分析丹参川芎嗪注射液治疗急性胰腺炎(AP)的效果及对血清淀粉酶(AMY)、脂肪酶(LPS)水平的影响。方法选取河南大学淮河医院2016年1月至2019年1月收治的96例AP病历资料,根据治疗方案分为两组,其中接受常规治疗方案的47例患者为对照组,基于对照组接受丹参川芎嗪注射液治疗的49例为研究组。对比两组疗效、临床症状改善时间、病情预测因子[AMY、LPS、二胺氧化酶(DAO)、D-乳酸(D-LAC)]、胃肠激素指标[胃动素(MTL)、血管活性肠肽(VIP)、胃泌素(GAS)]及不良反应。结果研究组总有效率高于对照组,腹痛腹胀消失、体温恢复正常、胰腺消肿、恢复进食、首次主动排便及住院时间均短于对照组,差异有统计学意义(P<0.05);治疗后研究组MTL高于对照组,AMY、LPS、DAO、D-LAC、VIP、GAS低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论丹参川芎嗪注射液用于治疗AP效果确切,可改善胃肠功能,促进临床症状恢复正常,安全性较高。Objective To retrospectively analyze the efficacy of Danshen Chuanxiongqin Injection in the treatment of acute pancreatitis(AP)and its impact on serum amylase(AMY)and lipase(LPS)levels.Methods The medical records of 96 cases of AP admitted to Huaihe Hospital of Henan University from January 2016 to January 2019 were selected and divided into two groups according to the treatment program,of which 47 cases treated with conventional treatment program were the control group,and 49 cases treated with Danshen Chuanxiongqin Injection based on the control group were the study group.The efficacy,time to improvement of clinical symptoms,predictive factors of disease[AMY,LPS,diamine oxidase(DAO),D-lactate(D-LAC)],gastrointestinal hormone indicators[motilin(MTL),vasoactive intestinal peptide(VIP),gastrin(GAS)],and adverse reactions were compared between two groups.Results The overall effective rate of the study group was higher than that of the control group,with disappearance of abdominal pain and bloating,normal body temperature,pancreatic edema reduction,resumption of eating,first active bowel movement,and hospital stay were shorter compared to the control group,and the differences were statistically significant(P<0.05).After treatment,the MTL of the study group was higher than that of the control group,while the levels of AMY,LPS,DAO,D-LAC,VIP,and GAS were lower than those of the control group,with statistical significance(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The use of Danshen Chuanxiongqin Injection for the treatment of AP is effective,can improve gastrointestinal function,promote clinical symptoms to return to normal,and has high safety.
