活血化瘀汤结合动态关节松动术在全膝关节置换术患者中的应用效果

摘　　要：目的观察活血化瘀汤结合动态关节松动术在全膝关节置换术患者中的应用效果。方法选取2019年5月至2022年5月河南中医药大学第五临床医学院收治的200例全膝关节置换术患者,分为研究组和对照组,各100例。对照组接受动态关节松动术康复锻炼,研究组接受活血化瘀汤辅助动态关节松动术康复锻炼。比较两组患者的疼痛程度、生活质量评分、膝关节关节活动度、Lysholm膝关节评分、股四头肌Kendall肌力评分、炎症因子水平、住院期间镇痛药使用情况等。结果治疗后,两组患者的Lysholm评分、疼痛评分、关节活动度、Kendall肌力评分和生活质量差异有统计学意义(P<0.05);研究组患者的白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平低于对照组(P<0.05);研究组患者吗啡注射液注射次数少于对照组,塞来昔布服用粒数少于对照组(P<0.05)。结论活血化瘀汤结合动态关节松动术可减轻全膝关节置换术后患者的疼痛程度,促进膝关节功能的恢复。Objective To observe the application effect of Huoxue Huayu Decoction combined with dynamic joint loosening surgery in patients undergoing total knee arthroplasty.Methods A total of 200 patients who underwent total knee arthroplasty admitted to the Fifth Clinical Medical College of Henan University of Traditional Chinese Medicine from May 2019 to May 2022 were selected and divided into a study group and a control group,with 100 patients in each group.The control group received dynamic joint mobilization rehabilitation exercise,while the study group received Huoxue Huayu Decoction assisted dynamic joint mobilization rehabilitation exercise.The pain level,quality of life score,knee joint range of motion,Lysholm knee joint score,Kendall quadriceps muscle strength score,levels of inflammatory factors,and use of analgesics during hospitalization were compared between two groups of patients.Results After treatment,there were statistically differences in Lysholm score,pain score,joint range of motion,Kendall muscle strength score,and quality of life scores between the two groups of patients(P<0.05).The levels of interleukin-6(IL-6)and interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)in the study group of patients were lower than those in the control group(P<0.05).The number of injections of morphine injection in the study group was less than that in the control group,and the number of tablets taken with celecoxib was less than that in the control group(P<0.05).Conclusion The combination of Huoxue Huayu Decoction and dynamic joint mobilization surgery can alleviate the pain level of patients after total knee arthroplasty and promote the recovery of knee joint function.
