摘  要:目的探讨血清微小RNA-34a(miR-34a)、Toll样受体4(TLR4)对稳定期慢性阻塞性肺疾病(SCOPD)患者预后的预测效能。方法回顾性分析2022年2月至2023年2月收治的SCOPD患者107例,对患者进行1年随访,根据患者临床预后分为预后良好组41例及预后不良组66例,并收集患者临床资料,分析导致不良预后发生的影响因素,并评估血清miR-34a、TLR4水平对患者不良预后的预测效能。结果预后不良组FVC、FEV1及FEV1/FVC水平均低于预后良好组,血清miR-34a、TLR4水平均高于预后良好组(P<0.01)。FVC、FEV1、FEV1/FVC、miR-34a、TLR4水平均为患者出现不良预后的影响因素(P<0.05,P<0.01)。miR-34a联合TLR4预测SCOPD不良预后的曲线下面积为0.854,敏感度、特异度分别为86.36%、78.05%(P<0.01)。结论miR-34a、TLR4水平均为SCOPD患者不良预后的影响因素,是预测患者不良预后的有效指标。Objective To investigate the predictive efficacy of serum microRNA-34a(miR-34a)and Toll-like receptor 4(TLR4)on the prognosis of patients with stable chronic obstructive pulmonary disease(SCOPD).Methods A total of 107 patients with SCOPD admitted from February 2022 to February 2023 were retrospectively analyzed and followed up for 1 year.According to the clinical prognosis,the patients were divided into the good prognosis group(n=41)and the poor prognosis group(n=66),and the clinical data of the patients were collected to analyze the influencing factors leading to the occurrence of poor prognosis.The predictive efficacy of serum miR-34a and TLR4 levels for poor prognosis of patients was evaluated.Results Compared with the good prognosis group,the poor prognosis group had significantly lower levels of FVC,FEV1,and FEV1/FVC and significantly higher levels of serum miR-34a and TLR4(P<0.01).FVC,FEV1,FEV1/FVC,miR-34a and TLR4 levels were all influencing factors for poor prognosis of patients(P<0.05,P<0.01).The area under the curve of miR-34a combined with TLR4 to predict poor prognosis of SCOPD was 0.854,and the sensitivity and specificity were 86.36%and 78.05%,respectively(P<0.01).Conclusion The levels of miR-34a and TLR4 are both influencing factors for the poor prognosis of patients with SCOPD,and they are effective indicators to predict the poor prognosis of patients.
