清热解毒化痰泻肺汤联合阿奇霉素治疗儿童痰热闭肺证肺炎支原体肺炎临床效果观察

摘　　要：目的探讨清热解毒化痰泻肺汤联合阿奇霉素治疗儿童痰热闭肺证肺炎支原体肺炎(MPP)的临床效果及对症状体征积分、炎性因子的影响。方法选取2023年10月至2024年6月收治的中医辨证为痰热闭肺证MPP患儿96例,随机数字表法分为对照组和观察组各48例,2组均给予常规治疗,对照组使用阿奇霉素治疗,观察组在对照组基础上加服清热解毒化痰泻肺汤治疗。观察2组临床疗效,治疗后症状体征积分、炎症指标(C反应蛋白、白细胞介素-6及肿瘤坏死因子-α)、胸CT病变改善情况,观察治疗期间2组不良反应发生情况。结果观察组总有效率[95.83%(46/48)]高于对照组[79.17%(38/48)],差异有统计学意义(P<0.05)。治疗后,2组症状、体征及总积分、炎症指标水平均较治疗前下降,且观察组低于对照组(P<0.05,P<0.01)。治疗后,2组肺实变、肺不张、支气管狭窄、胸腔积液患者总占比低于对照组,且观察组低于对照组(P<0.05)。2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论清热解毒化痰泻肺汤联合阿奇霉素治疗儿童痰热闭肺证MPP能有效缓解临床症状,减轻炎症反应,促进胸部病变恢复,提高临床疗效,且安全性较高。Objective To investigate the clinical effect of Qingre Jiedu Huatan Xiefei decoction combined with Azithromycin in the treatment of mycoplasma pneumoniae pneumonia(MPP)in children with phlegm-heat blocking lung syndrome and their impact on the scores of symptoms and signs and inflammatory factors.Methods In total,96 MPP children with phlegm-heat blocking lung syndrome treated from October 2023 to June 2024 were selected and divided into the control group(n=48)and the observation group(n=48)according to random number table method.Both groups were given conventional treatment,the control group was treated with Azithromycin,and the observation group was treated with Qingre Jiedu Huatan Xiefei decoction on the basis of the control group.The clinical efficacy,scores of symptoms and signs,inflammatory indicators[C reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)]and improvement of chest CT lesions after treatment were observed in the two groups,and the occurrence of adverse reactions during treatment was observed.Results The total effective rate of the observation group[95.83%(46/48)]was higher than that of the control group[79.17%(38/48)],and the difference was statistically significant(P<0.05).After treatment,the scores of symptoms and signs,total scores and inflammatory indicators in the two groups were lower than those before treatment,and lower in the observation group than in the control group(P<0.05,P<0.01).After treatment,the total proportion of patients with lung consolidation,atelectasis,bronchial stenosis,and pleural effusion in both groups was lower than that in the control group,which was lower in the observation group than in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion In the treatment of MPP in children with phlegm-heat blocking lung syndrome,Qingre Jiedu Huatan Xiefei decoction combined with Azithromycin can effectively relieve clinical symptoms,reduce inflammatory reactio
