摘  要:目的:分析血清视黄醇结合蛋白(RBP)、胱抑素C(CysC)、降钙素原(PCT)及C反应蛋白(CRP)表达水平与慢性阻塞性肺疾病(COPD)严重程度之间的关系。方法:选取佳木斯大学附属第一医院自2023年10月至2024年4月在呼吸与危重症医学科就诊的62例COPD患者。根据肺功能相关指标(第1秒用力呼气容积占预计值百分比,FEV1%pred)将COPD患者分为两组,轻中度组(FEV1%pred≥50%,30例)和重及极重度组(FEV1%pred<50%,32例)。另选取同一时间段佳木斯大学附属第一医院健康体检者30例纳入对照组。分别测定FEV1%pred及FEV1/FVC以及血清RBP、CysC、PCT及CRP表达水平。采用受试者工作特征(ROC)曲线分析血清RBP、CysC、PCT联合CRP对重及极重度COPD患者的预测效能。结果:COPD组血清CysC、PCT及CRP表达水平显著高于对照组(P<0.05),RBP、FEV1%pred及FEV1/FVC显著低于对照组(P<0.05)。与轻中度组比较,重及极重度组血清CysC、PCT及CRP表达水平显著升高(P<0.05),RBP、FEV1%pred及FEV1/FVC显著降低(P<0.05)。COPD患者血清RBP、CysC、PCT及CRP的表达水平与FEV1%pred及FEV1/FVC有一定相关性(P<0.05);COPD组患者FEV1%pred及FEV1/FVC与血清CysC、PCT及CRP的表达水平呈负相关(P<0.05),与BRP呈正相关(P<0.05)。ROC曲线显示,RBP、CysC、PCT及CRP四项联合检测对预测重及极重度COPD患者的曲线面积为0.930,灵敏度和特异度可达到90.0%、86.7%。结论:血清RBP、CysC、PCT及CRP表达水平与COPD患者肺功能密切相关,多指标联合检测对预测重及极重度COPD患者具有一定的价值。Objective:To analyze the relationship between the expression levels of serum retinol binding protein(RBP),cystatin C(CysC),procalcitonin(PCT),C-reactive protein(CRP)and the severity of chronic obstructive pulmonary disease(COPD).Methods:62 COPD patients admitted to the Department of Respiratory and Critical Care Medicine of The First Affiliated Hospital of Jiamusi University from October 2023 to April 2024 were enrolled.Based on lung function indicators(percentage of forced expiratory volume in the first second as a percentage of predicted,FEV1%pred),the COPD patients were divided into two groups:a mild-to-moderate group(FEV1%pred≥50%,30 patients)and a severe-to-very severe group(FEV1%pred<50%,32 patients).Additionally,30 healthy individuals from the same hospital during the same period were selected as the control group.FEV1%pred,FEV1/FVC,and serum levels of RBP,CysC,PCT and CRP were measured.Receiver operating characteristic(ROC)curves were used to analyze the predictive efficacy of combined serum biomarkers RBP,CysC,PCT and CRP for severe-to-very severe COPD patients.Results:The serum levels of CysC,PCT and CRP in the COPD group were significantly higher than those in the control group,while the levels of RBP,FEV1%pred,and FEV1/FVC were significantly lower(P<0.05).Compared with the mild-to-moderate group,the severe-to-very severe group had significantly higher serum levels of CysC,PCT and CRP(P<0.05)and significantly lower levels of RBP,FEV1%pred and FEV1/FVC(P<0.05).The expression levels of serum RBP,CysC,PCT and CRP in COPD patients were correlated with FEV1%pred and FEV1/FVC(P<0.05);In the COPD group,FEV1%pred and FEV1/FVC were negatively correlated with the expression levels of serum CysC,PCT,and CRP(P<0.05)and positively correlated with RBP(P<0.05).The ROC curve showed that the combined detection of RBP,CysC,PCT,and CRP had an area under the curve of 0.930 for predicting severe-to-very severe COPD patients,with sensitivity and specificity reaching 90.0%and 86.7%,respectively.Conclusion:The expression le
