安珂微创引流术与常规切开引流术治疗乳腺脓肿的对比研究

摘　　要：目的:分析安珂(EnCor)微创引流术与常规切开引流术治疗乳腺脓肿的效果。方法:回顾性选取2021年11月至2023年10月许昌市中心医院收治的60例乳腺脓肿患者临床资料,根据不同手术方案分为两组,其中行EnCor微创引流术治疗的30例为研究组,行常规切开引流术治疗的30例为对照组。对比两组围术期指标、疼痛度、美观度、炎性因子水平[白细胞介素-2(IL-2)、IL-4、细胞间黏附分子1(ICAM-1)、肿瘤坏死因子α(TNF-α)]及并发症发生率。结果:研究组术中出血量、换药次数少于对照组,切口长度、手术时间、抗生素使用时间、切口愈合时间及住院时间均短于对照组(P<0.05);术后1 d、3 d、7 d研究组疼痛评分、美观评分低于对照组(P<0.05);术后7 d研究组ICAM-1、IL-2、TNF-α低于对照组,IL-4高于对照组(P<0.05);两组并发症发生率对比,差异无统计学意义(P>0.05)。结论:EnCor微创引流术治疗乳腺脓肿手术快捷、创伤小、术后恢复快,可抑制炎性反应,减轻疼痛,改善术后美观度。Objective:To analyze the effectiveness of EnCor minimally invasive drainage and conventional incision and drainage in the treatment of breast abscess.Methods:A retrospective analysis was conducted on the clinical data of 60 patients with breast abscess admitted to Xuchang Central Hospital from November 2021 to October 2023.The patients were divided into two groups based on different surgical approaches:the study group(30 patients)underwent EnCor minimally invasive drainage,and the control group(30 patients)underwent conventional incision and drainage.The perioperative indicators,pain intensity,cosmetic outcome,levels of inflammatory cytokines[interleukin-2(IL-2),IL-4,intercellular adhesion molecule-1(ICAM-1),tumor necrosis factor-α(TNF-α)],and complication rates were compared between the two groups.Results:The study group had less intraoperative blood loss and fewer dressing changes compared with the control group.The incision length,surgery duration,antibiotic use duration,incision healing time and hospital stay were shorter in the study group than in the control group(P<0.05).The pain scores and cosmetic scores were lower in the study group than in the control group at 1 day,3 days and 7 days postoperatively(P<0.05).At 7 days postoperatively,the levels of ICAM-1,IL-2 and TNF-αwere lower in the study group than in the control group,while the level of IL-4 was higher(P<0.05).There was no statistically significant difference in the complication rates between the two groups(P>0.05).Conclusion:EnCor minimally invasive drainage for breast abscess is a rapid and less invasive surgery with fast postoperative recovery.It can inhibit inflammatory responses,reduce pain and improve postoperative cosmetic outcomes.
