摘 要:目的:探究卡格列净联合厄贝沙坦对早期糖尿病肾病(DN)患者尿蛋白、氧化应激指标的影响。方法:选取项城市中医院2021年1月至2023年12月收治的118例早期DN患者,以不同治疗方案分为A、B两组,各59例,两组均应用二甲双胍治疗,在此基础上分别应用厄贝沙坦、厄贝沙坦联合卡格列净治疗,比较两组治疗前后尿蛋白指标、肾功能指标、氧化应激损伤指标及不良反应。结果:治疗后B组24 h尿蛋白定量、尿微量白蛋白排泄率水平均较A组低,血肌酐(Scr)、尿素氮(BUN)水平较A组低,丙二醛(MDA)、总抗氧化能力(T-AOC)水平较A组低,超氧化物歧化酶(SOD)水平较A组高(P<0.05);B组不良反应发生率与A组相近(P>0.05)。结论:对早期DN患者应用卡格列净联合厄贝沙坦治疗,可改善其尿蛋白水平,减轻肾功能损伤及氧化应激反应,且安全性良好。Objective:To explore the effect of canagliflozin combined with irbesartan on urinary protein and oxidative stress indexes in patients with early diabetic nephropathy(DN).Methods:118 patients with early DN from Xiangcheng Traditional Chinese Medicine Hospital of Henan Province between January 2021 and December 2023 were divided into two groups,A and B,with 59 patients in each group,based on different treatment regimens.Both groups received metformin treatment as the basis,with Group A additionally receiving irbesartan and Group B receiving a combination of irbesartan and canagliflozin.The urinary protein indices,renal function indices,oxidative stress damage indices,and adverse reactions were compared between the two groups before and after treatment.Results:After treatment,Group B had lower levels of 24-hour urinary protein quantitation,urinary microalbumin excretion rate,serum creatinine(Scr),urea nitrogen(BUN),malondialdehyde(MDA)and total antioxidant capacity(T-AOC)compared to Group A,while the level of superoxide dismutase(SOD)was higher in Group B than in Group A(P<0.05).The incidence of adverse reactions in Group B was similar to that in Group A(P>0.05).Conclusion:The combination of canagliflozin and irbesartan can improve urinary protein levels,reduce renal injury and oxidative stress in patients with early DN,with good safety.
