分娩期体位护理对初产妇足月头位未衔接胎膜早破中应用效果及其对产后心理状态的影响

摘　　要：目的:探讨分娩期体位护理对初产妇足月头位未衔接胎膜早破(premature rupture of membranes,PROM)中应用效果及其对产后心理状态的影响。方法:回顾性选取2021年1月至2023年1月收治的60例于天水市妇幼保健院进行生产的足月头位未衔接PROM初产妇作为研究对象,按照分娩护理方式分组,给予传统护理的30例产妇为对照组,给予分娩期体位护理的30例产妇为观察组。观察两组产妇产程时间、妊娠结局、产妇舒适度及心理状态。结果:观察组自然分娩率高于对照组,第一、二、三产程较对照组短(P<0.05);观察组产妇分娩舒适状况量表(general comfort questionnaire,GCQ)评分较对照组高,视觉模拟疼痛量表(visual analogue scale/score,VAS)较对照组低(P<0.05);产后观察组产妇汉密尔顿抑郁量表(hamilton depression scale,HAMD)及汉密尔顿焦虑量表(hamilton anxiety scale,HAMA)评分较对照组降低(P<0.05)。结论:对足月头位未衔接PROM的初产妇采用分娩期体位护理可有效缩短产程时间,增加自然分娩率,提高产妇的分娩舒适度,改善其心理状态。Objective:To explore t he application effect of body position nursing during labor on premature rupture of membranes(PROM)in primiparas with unengaged cephalic presentation at full term and its influence on postpartum psychological status.Methods:A retrospective study was conducted on 60 primiparas with unengaged cephalic presentation and PROM who delivered at Tianshui Maternal and Child Health Hospital between January 2021 and January 2023.The participants were divided into two groups based on their mode of delivery care:the control group of 30 primiparas received traditional care,while the observation group of 30 primiparas received body position nursing during delivery.The duration of labor,pregnancy outcomes,maternal comfort and psychological status of the two groups were observed.Results:The natural delivery rate in the observation group was higher than that in the control group,and the durations of the first,second and third stages of labor were shorter in the observation group compared to the control group(P<0.05).The General Comfort Questionnaire(GCQ)scores of the parturients in the observation group were higher,while the Visual Analogue Scale/Score(VAS)scores were lower than those in the control group(P<0.05).Additionally,the Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)scores of the parturients in the observation group were lower than those in the control group after delivery(P<0.05).Conclusion:Body position nursing during labor for primiparas with unengaged cephalic presentation and PROM at full term can effectively shorten the duration of labor,increase the rate of spontaneous delivery,enhance maternal comfort during childbirth,and improve their psychological state.
