自拟消肿汤在髋部骨折术后肢体肿胀及下肢DVT形成防治中的应用

摘　　要：目的:探讨自拟消肿汤在髋部骨折术后肢体肿胀及下肢深静脉血栓(deep vein thrombosis,DVT)形成防治中的应用效果。方法:选取2022年1月至2023年1月于邵武市立医院诊治髋部骨折患者104例,随机分为两组:对照组52例采用常规西医治疗,观察组52例则在此基础上辅以自拟消肿汤治疗。比较两组中医主要症状评分、患侧肢体肿胀程度、血流动力学指标水平、髋关节Harris评分、骨密度、术后3个月下肢DVT发生率及不良反应发生情况。结果:两组术后1个月及3个月主要中医症状评分、全血高切黏度、全血低切黏度及血浆黏度水平较治疗前明显下降,髋关节Harris评分和骨密度水平均较治疗前明显提高,观察组术后以上指标均明显优于同期对照组(P<0.05)。两组术后3 d及7 d患侧肢体肿胀程度较治疗前明显下降,观察组术后以上指标均明显低于同期对照组(P<0.05)。观察组术后3个月下肢DVT发生率显著低于对照组(P<0.05);同时两组治疗过程中均未见严重不良反应。结论:自拟消肿汤辅助用于髋部骨折患者可有效缓解临床症状,减轻术后肢体肿胀程度,改善血流动力学指标,预防下肢DVT发生,且安全性良好。Objective:To explore the application effect of self-designed detumescence decoction in the prevention and treatment of limb swelling and lower limb DVT formation after hip fracture surgery.Methods:104 patients with hip fractures diagnosed and treated at Shaowu Municipal Hospital in Fujian Province between January 2022 and January 2023 were randomly divided into two groups:the control group 52 patients received conventional Western Medicine treatment,while the observation group 52 patients`received additional treatment with self-designed detumescence decoction on this basis.The main Traditional Chinese Medicine symptoms scores,limb swelling degree on the affected side,hemodynamic index levels,Harris hip score,bone density,incidence of lower limb DVT 3 months after surgery and adverse reactions were compared between the two groups.Results:1 month and 3 months after surgery,the scores for the main Traditional Chinese Medicine symptoms,whole blood high-shear viscosity,whole blood low-shear viscosity and plasma viscosity levels in both groups were significantly lower than before treatment.Additionally,Hip Joint Harris scores and bone mineral density levels were significantly higher than before treatment.The observation group showed significantly better improvements in these indicators compared to the control group at the same time points(P<0.05).3 and 7 days after surgery,the degree of swelling in the affected limb in both groups was significantly lower than before treatment,with the observation group showing significantly lower levels than the control group at the same time points(P<0.05).The incidence of lower limb DVT 3 months after surgery was significantly lower in the observation group compared to the control group(P<0.05).No serious adverse reactions were observed in either group during the treatment process.Conclusion:As an adjuvant therapy for hip fracture patients,self-designed detumescence decoction can effectively alleviate clinical symptoms,reduce the degree of postoperative limb swelling,improve hemodyna
