舒适护理模式对慢性阻塞性肺病患者护理效果的meta分析

摘　　要：目的评价舒适护理模式对慢性阻塞性肺病(COPD)患者的护理效果。方法在PubMed、维普、中国知网、万方数据检索舒适护理对慢性阻塞性肺病患者护理效果的临床研究,通过纳排标准最终纳入9项临床研究,样本量共840例,其中试验组420例、对照组420例,运用RevMan 5.3软件进meta分析。结果舒适护理对COPD患者焦虑症状评分(SMD=-3.92,95%CI:-4.98~-2.57,P<0.00001)、抑郁症状评分(SMD=-4.20,95%CI:-6.12~-2.28,P<0.00001)、生活质量评分(SMD=4.37,95%CI:2.74~6.00,P<0.00001)、肺功能指标第1秒用力呼气量(SMD=1.44,95%CI:0.88~2.00,P<0.00001)、肺功能指标第1秒用力肺活量(SMD=0.68,95%CI:0.36~1.00,P<0.00001)较常规护理具有显著的改善作用,同时亦能显著提升患者的满意度(OR=7.55,95%CI:3.58~15.90,P<0.0001),差异有统计学意义。结论舒适护理模式对COPD患者的护理效果较常规护理更有优势。Objective To evaluate the nursing effect of comfort nursing mode on patients with chronic obstructive pulmonary disease(COPD).Methods In the clinical studies on the nursing effect of comfort nursing on patients with COPD retrieved from PubMed,VIP,CNKI and Wanfang data,9 clinical studies were finally included through the exclusion standard,with a total sample size of 840 cases,including 420 cases in the experimental group and 420 cases in the control group.meta-analysis was performed using RevMan 5.3 software.Results Comfort nursing in COPD patients with anxiety symptom score(SMD=-3.92,95%CI:-4.98--2.57,P<0.00001),depression symptom score(SMD=-4.20,95%CI:-6.12--2.28,P<0.00001),quality of life score(SMD=4.37,95%CI:2.74-6.00,P<0.00001),pulmonary function index forced expiratory volume in the first second(SMD=1.44,95%CI:0.88-2.00,P<0.00001),the pulmonary function index forced vital capacity in the first second(SMD=0.68,95%CI:0.36-1.00,P<0.00001)significantly improved compared with conventional care,and also significantly improved patient satisfaction(OR=7.55,95%CI:3.58-15.90,P<0.0001),the differences were statistically significant.Conclusion Comfort nursing mode has more advantages in nursing effect of COPD patients than conventional nursing.
