绝经后女性良性阵发性位置性眩晕发生的影响因素

摘　　要：目的探讨绝经后女性良性阵发性位置性眩晕(BPPV)发生的影响因素。方法选取2023年1月至2024年5月上饶市人民医院收治的60例绝经后女性BPPV患者作为研究组,选取同期接受健康体检的60例绝经后女性作为对照组,对所有女性进行一般资料比较,对绝经后女性BPPV发生进行多因素logistic回归分析。结果单因素分析结果显示,两组患者的年龄、体重指数(BMI)、高血压病史、糖尿病病史、冠心病病史、空腹血糖(FBG)、高密度脂蛋白胆固醇(HDL-C)、收缩压(SBP)、舒张压(DBP)比较,差异无统计学意义(P>0.05);两组患者的同型半胱氨酸、低密度脂蛋白胆固醇(LDL-C)、白蛋白(ALB)水平比较,差异有统计学意义(P<0.05);多因素logistic分析结果显示,同型半胱氨酸水平(β=1.674,OR=5.335,95%CI:4.379~6.341)是影响绝经后女性BPPV发生的独立危险因素(P<0.05)。结论血清同型半胱氨酸水平在绝经后女性BPPV患者中表达较高,同型半胱氨酸水平为绝经后女性BPPV发生的独立危险因素。Objective To explore the influencing factors of benign paroxysmal positional vertigo(BPPV)in postmenopausal women.Methods A total of 60 postmenopausal female BPPV patients admitted to Shangrao People's Hospital from January 2023 to May 2024 were selected as the study group,and 60 postmenopausal women who underwent physical examination during the same period were selected as the control group.After comparison of general data of all women,multivariate logistic regression analysis was performed on the occurrence of BPPV in postmenopausal women.Results Univariate analysis showed that there were no significant differences in age,body mass index(BMI),history of hypertension,diabetes,coronary heart disease,fasting blood glucose(FBG),high-density lipoprotein cholesterol(HDL-C),systolic blood pressure(SBP)and diastolic blood pressure(DBP)between the two groups(P>0.05).The levels of homocysteine,low-density lipoprotein cholesterol(LDL-C)and albumin(ALB)in two group were significantly different(P<0.05).The results of multivariate analysis suggested that homocysteine level(β=1.674,OR=5.335,95%CI:4.379-6.341)was an independent risk factor for BPPV in postmenopausal women(P<0.05).Conclusion Serum homocysteine level is highly expressed in postmenopausal women with BPPV,and homocysteine level is an independent risk factor for BPPV in postmenopausal women.
