大黄芒硝散外敷在重症心力衰竭合并胃肠功能障碍患者中的应用效果

摘　　要：目的探讨大黄芒硝散外敷在重症心力衰竭(SHF)合并胃肠功能障碍患者中的应用效果。方法选取2023年1月至12月江西省人民医院收治的68例SHF合并胃肠功能障碍患者作为研究对象,采用随机数字表法分为对照组和观察组,每组34例。对照组予以常规治疗及护理,观察组在此基础上予以大黄芒硝散外敷,干预5 d。比较两组胃肠功能障碍评分、腹内压、病情严重程度、胃肠功能恢复情况、营养支持时间、胃肠道临床症状量化积分及护理满意度。结果观察组干预后胃肠功能障碍评分、腹内压、急性生理与慢性健康评分Ⅱ(APACHE-Ⅱ)评分、胃肠道临床症状量化积分低于对照组,差异有统计学意义(P<0.05);观察组肠功能恢复、重症加强护理病房(ICU)入住及住院时间短于对照组,肠内营养(EN)、肠外营养(PN)、PN+EN支持时间短于对照组,护理满意度高于对照组,差异有统计学意义(P<0.05)。结论在SHF合并胃肠功能障碍患者中应用大黄芒硝散外敷可促进胃肠功能恢复,改善患者病情,提高护理满意度,值得临床借鉴。Objective To explore the application effect of external application of Dahuang Mirabilite Powder in patients with severe heart failure(SHF)complicated with gastrointestinal dysfunction.Methods A total of 68 SHF complicated with gastrointestinal dysfunction patients admitted to Jiangxi Provincial People's Hospital from January to December 2023 were selected as the study objects,and they were divided into control group and observation group by random number table method,with 34 cases in each group.The control group received routine treatment and nursing,while the observation group received external application of Dahuang Mirabilite Powder on this basis.The intervention lasted for 5 days,and the gastrointestinal dysfunction score,intra-abdominal pressure,severity of the condition,recovery of gastrointestinal function,nutritional support time and nursing satisfaction were compared between the two groups.Results After intervention,the gastrointestinal dysfunction score,intra-abdominal pressure,acute physiological and chronic health scoreⅡ(APACHE-Ⅱ)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The recovery of intestinal function,ICU stay and hospital stay in the observation group were shorter than those in the control group,while enteral nutrition(EN),parenteral nutrition(PN),and PN+EN support time were shorter than those in the control group,the nursing satisfaction was higher in the observation group than that in the control group,and the differences were statistically significant(P<0.05).Conclusion The application of Dahuang Mirabilite Powder in SHF complicated with gastrointestinal dysfunction patients can promote gastrointestinal function recovery,improve patient condition,and increase nursing satisfaction,which is worth clinical reference.
