循证护理在经肛全直肠系膜切除术后患者中的应用效果

摘　　要：目的分析基于循证护理方案设计在经肛全直肠系膜切除术(TaTME)术后患者中的应用效果。方法选取2021年5月至2023年7月厦门大学附属第一医院胃肠肿瘤外科接受TaTME治疗的130例直肠癌患者作为研究对象,采用随机单双数抽签法将其分为对照组和研究组,各65例。对照组予常规护理,研究组在常规护理实施基础上予以基于循证护理方案。比较两组术后疼痛评分、术后并发症情况及护理前后生活质量评价量表(SF-36)评分。结果研究组术后第2、4天的疼痛评分低于对照组,差异有统计学意义(P<0.05)。护理后,研究组护理后躯体功能、生命力、躯体角色、一般健康状况评分均高于对照组,差异有统计学意义(P<0.05)。研究组术后护理满意度评分高于对照组,差异有统计学意义(P<0.05)。研究组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论基于循证护理方案设计在TaTME后直肠癌患者中的应用可有效预防术后并发症、缓解术后疼痛,提升患者生活质量,值得应用。Objective To analyze the application effect of evidence-based nursing program design in patients after transanal total mesorectal resection(TaTME).Methods A total of 130 patients with rectal cancer treated with TaTME in the Department of Gastrointestinal Oncology,the First Affiliated Hospital of Xiamen University from May 2021 to July 2023 were selected as the research objects.According to the random odd and even drawing method,they were divided into the control group and the study group,with 65 cases in each group.The control group was given routine nursing,and the study group was given evidence-based nursing program design on the basis of routine nursing.The postoperative pain score,postoperative complications and the MOS item short form health survey(SF-36)score before and after nursing were compared between the two groups.Results The pain scores of the study group were lower than those of the control group on the 2nd and 4th day after surgery,and the differences were statistically significant(P<0.05).After nursing,the scores of physical function,vitality,physical role and general health status of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).The score of nursing satisfaction after operation of study group was higher than that of the control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Application of the evidence-based nursing program design in rectal cancer patients after TaTME can effectively prevent postoperative complications,relieve postoperative pain,and improve the quality of life of patients,which is worthy of application.
