摘　　要：幽门螺杆菌(Hp)感染是最常见的慢性感染之一,它与多种消化系统疾病如慢性胃炎、消化性溃疡、胃癌及胃黏膜相关性淋巴组织(MALT)淋巴瘤等关系密切。Hp感染的药物根除治疗在全世界范围内已广泛开展。目前Hp感染的现状是高感染率、高耐药率、高致病力和低根除率。既往常用的“三联”或“四联”疗法的缺点是药物的副作用大、费用昂贵、耐药菌株不断增加,以及患者的依从性差,这些问题导致治疗效果变差,因此国内外研究者在Hp的药物治疗方面进行了不断探索。本文综述近年在Hp药物治疗方面的治疗指征的变化、抗生素耐药情况与用药方案的改进、治疗失败后的方案选择和几种新型治疗策略及药物等研究进展,为临床药物治疗Hp感染提供参考。Helicobacter pylori(Hp)infection is one of the most common chronic infections,it is closely related to a variety of digestive diseases,such as chronic gastritis,peptic ulcer,gastric cancer and gastric muco-associated lymphoid tissue(MALT)lymphoma.The drug eradication of Hp infection has been widely carried out in the world.The current status of Hp infection is high infection rate,high drug resistance rate,high pathogenicity and low eradication rate.The disadvantages of"triple"or"quadruple"therapy commonly used in the past are large side effects of drugs,high cost,increasing drug-resistant strains and poor patient compliance,which lead to poor treatment effect,so domestic and foreign researchers have carried out continuous exploration in the drug treatment of Hp.In this paper,the changes of therapeutic indications,the improvement of antibiotic resistance and drug regimen,the selection of regimen after treatment failure,and several novel therapeutic strategies and drugs were reviewed in recent years,so as to provide references for clinical drug treatment of Hp infection.
