曲安奈德与A型肉毒毒素联合治疗对增生性瘢痕患者的疗效观察

摘　　要：目的探讨曲安奈德联合A型肉毒毒素治疗增生性瘢痕(HS)患者的影响。方法选择2021年6月至2024年6月新乡市中心医院收治的HS患者84例,按治疗方法不同分为2组,各42例。对照组患者予以曲安奈德注射液治疗,观察组患者予以曲安奈德注射液联合A型肉毒毒素治疗。比较两组患者的临床疗效、血清细胞因子水平、瘢痕指标、疼痛程度、不良反应。结果治疗后,观察组总有效率95.24%、骨形态发生蛋白7(335.94±20.39)ng/L均高于对照组80.95%、(304.59±18.86)ng/L,血清转化生长因子(189.62±12.74)ng/L、温哥华瘢痕量表中色泽(0.86±0.20)分、(1.42±0.36)分、(0.92±0.24)分、(1.74±0.36)分、视觉模拟评分法(3.62±0.37)分均低于对照组(237.54±16.85)ng/L、(1.62±0.39)分、(2.05±0.48)分、(1.58±0.37)分、(2.69±0.50)分、(4.94±0.59)分,差异有统计学意义(P<0.05);两组间不良反应相当,差异无统计学意义(P>0.05)。结论曲安奈德联合A型肉毒毒素治疗HS患者的疗效显著,能够改善血清细胞因子水平及瘢痕,减轻疼痛程度,且不良反应少。Objective To investigate the effect of triamcinolone and botox type A on the treatment of hypertrophic scar(HS).Methods A total of 84 patients with HS admitted to Xinxiang Central Hospital from June 2021 to June 2024 were selected and divided into 2 groups with 42 cases in each group according to the treatment method is different.The control group was treated with triamcinolone injection,and the observation group was treated with triamcinolone injection combined with botox type A.The clinical effificacy,serum cytokine level,scar index,pain degree and adverse reactions were compared between the two groups.Results After treatment,the total effective rate 95.24%and bone morphogenetic protein 7(335.94±20.39)ng/L in the observation group were higher than those in the control group 80.95%and(304.59±18.86)ng/L.Serum transforming growth factor(189.62±12.74)ng/L,Vancouver Scar Scale color(0.86±0.20),(1.42±0.36),(0.92±0.24),(1.74±0.36),visual analogue scale(3.62±0.37)score was lower than that of control group(237.54±16.85)ng/L,(1.62±0.39),(2.05±0.48),(1.58±0.37),(2.69±0.50),(4.94±0.59),and there was statistical signifificance(P<0.05).There was no statistical signifificance in adverse reactions between the two groups(P>0.05).Conclusions Triamcinolone and botox A are effective in the treatment of HS patients,which can improve the level of serum cytokines and scar,reduce the degree of pain,and have few adverse reactions.
