摘  要:目的研究免疫组织化学增强二抗法在骨髓瘤骨髓活检中检测κ、λ蛋白表达的应用价值。方法收集上海中医药大学附属曙光医院病理科2022年7月—2023年7月期间送检的20例多发性骨髓瘤病例。根据病理形态学诊断,并结合常规免疫组织化学检测结果,其中10例诊断为多发性骨髓瘤(呈限制性κ表达),10例诊断为多发性骨髓瘤(呈限制性λ表达)。在调整染色方案和处理细节后,分别对20例病例的连续切片分组,分别采用普通二抗法和增强二抗法进行免疫组织化学检测,比较两种方法在表达检测中的效果。结果增强二抗法在骨髓瘤骨髓活检组织中对κ、λ蛋白的检测阳性率高于普通二抗法,且增强二抗法的染色定位更加清晰,背景干净,染色效果优良。结论免疫组织化学增强二抗法检测骨髓瘤骨髓活检中κ、λ蛋白的表达,表现出更高的灵敏度,能够在骨髓瘤分型判读中提供帮助,并具有提高骨髓瘤骨髓活检检出率的潜力。Objective To investigate the application value of immunohistochemistry enhanced secondary antibody method in detectingκandλprotein expression in bone marrow biopsies from multiple myeloma.Methods A total of 20 cases of multiple myeloma were collected from the pathology department of Shuguang Hospital affiliated with Shanghai University of Traditional Chinese Medicine,with samples sent for testing between July 2022 and July 2023.Based on pathological morphological diagnosis and conventional immunohistochemical results,10 cases were diagnosed as multiple myeloma with restrictedκexpression,and 10 cases as multiple myeloma with restrictedλexpression.After adjusting the staining protocol and processing details,consecutive tissue sections from the 20 cases were grouped and tested with the conventional second antibody method and the enhanced second antibody method for immunohistochemistry,respectively.The expression detection results of the two methods were compared and analyzed.Results The enhanced second antibody method showed a higher positive detection rate forκandλproteins in bone marrow biopsy tissues from multiple myeloma,compared to the conventional second antibody method.Furthermore,the enhanced second antibody method exhibited clearer staining localization,cleaner background,and superior staining quality.Conclusion The enhanced second antibody method immunohistochemistry for detectingκandλprotein expression in multiple myeloma bone marrow biopsies demonstrates higher sensitivity,which can assist in the interpretation of multiple myeloma subtyping and has the potential to improve the detection rate of bone marrow biopsies in multiple myeloma.
