补气益肾祛瘀汤治疗肾虚血瘀型抗心磷脂抗体阳性复发性流产临床研究

摘　　要：目的:观察补气益肾祛瘀汤治疗肾虚血瘀型抗心磷脂抗体(anti cardiolipin antibody,ACA)阳性复发性流产(recurrent spontaneous abortion,RSA)的临床疗效。方法:选取2020年1月至2023年6月周口市中医院收治的肾虚血瘀型ACA阳性RSA患者96例,按照随机数字表法分为对照组和观察组,每组48例。对照组给予阿司匹林肠溶片联合醋酸泼尼松片治疗,观察组在对照组治疗的基础上加用补气益肾祛瘀汤治疗。观察两组ACA-免疫球蛋白(immunoglobulin,Ig)G滴度值、ACA-IgM滴度值、ACA-IgA滴度值、中医证候积分、临床疗效及安全性。结果:两组中医证候疗效比较,观察组有效率为93.75%,对照组有效率为79.17%,观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗1、2、3个疗程后中医证候积分低于对照组,差异有统计学意义(P<0.05);两组ACA-IgG疗效比较,观察组有效率为95.83%,对照组有效率为83.33%,观察组高于对照组,差异有统计学意义(P<0.05);观察组治疗3个疗程后ACA-IgG、ACA-IgM、ACA-IgA滴度值低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率为2.08%,对照组不良反应发生率为14.58%,观察组低于对照组,差异有统计学意义(P<0.05)。结论:补气益肾祛瘀汤治疗肾虚血瘀型抗心磷脂抗体阳性复发性流产,可显著提高临床疗效,有效调节患者机体免疫功能。Objective:To observe the clinical efficacy of Qi-Supplementing and Kidney-Tonifying and Stasis-Removing Decoction on recurrent spontaneous abortion(RSA)with positive anti-cardiolipin antibody(ACA)in kidney deficiency and blood stasis type.Methods:A total of 96 patients with ACA-positive RSA of kidney deficiency and blood stasis type admitted to Zhoukou Hospital of Traditional Chinese Medicine from January 2020 to June 2023 were selected and randomly divided into the control group and the observation group,with 48 cases in each group.The control group was treated with Enteric-Coated Aspirin Tablets combined with Prednisone Acetate Tablets,while the observation group was treated with Qi-Supplementing and Kidney-Tonifying and Stasis-Removing Decoction on the basis of the control group′s treatment.The ACA-IgG titer,ACA-IgM titer,ACA-IgA titer,TCM syndrome scores,clinical efficacy and safety were observed in both groups.Results:In terms of the therapeutic effect of TCM syndromes,the effective rate of the observation group was 93.75%,while that of the control group was 79.17%.The effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After 1,2,and 3 courses of treatment,the TCM syndrome scores of the observation group were lower than those of the control group,but the differences were not statistically significant(P<0.05).In terms of the therapeutic effect of ACA-IgG,the effective rate of the observation group was 95.83%,while that of the control group was 83.33%.The effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After three courses of treatment,the titer values of ACA-IgG,ACA-IgM,and ACA-IgA in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was 2.08%,while that in the control group was 14.58%.The incide
